Recurrent anterior dislocation of the hip: Case report with arthrographic findings

Abstract: A case of recurrent post-traumatic anterior dislocation of the hip in a 27-year-old woman is presented. Radiographs document this rare abnormality in the acute stage. Post-reduction arthrography demonstrates an anterior pseudobursa resulting from capsular tear.

“…Dancers' ligaments tend to be more lax due to repetitive flexibility training, which predispose them to hip joint instability. Guyer and Levinsohn [ 9 ] reported a case of anterior dislocation due to a low-energy fall. In these cases, all the patients showed some inherent quality that predisposed them to dislocation, including anterior capsule insufficiency, small center-edge angles, and developmental dysplasia.…”

“…In our search of the English literature, we found only 6 reported cases of recurrent anterior hip dislocation in adults and only 4 that occurred in young healthy patients. 5,10,11,17 All the reported dislocations were secondary to traumatic mechanisms.…”

“…Four cases of recurrent anterior hip dislocation in young healthy patients have been reported in the English literature and have been associated with anterior capsular redundancy. 5,10,12,17 Treatment options have included open capsulorrhaphy and either intertrochanteric derotational osteotomy or subtrochanteric femoral derotational osteotomy via an anterolateral approach. 10,17 In one report, a failed subtrochanteric femoral derotational osteotomy was successfully treated with a periacetabular osteotomy.…”
