Recurrent angina after coronary angioplasty: mechanisms, diagnostic and therapeutic options

Abstract: Recurrent angina in patients who underwent percutaneous coronary intervention is defined as recurrence of chest pain or chest discomfort. Careful assessment is recommended to differentiate between non-cardiac and cardiac causes. In the case of the latter, recurrent angina occurrence can be related to structural ('stretch pain', in-stent restenosis, in-stent thrombosis, incomplete revascularization, progression of coronary atherosclerosis) or functional (coronary micro-vascular dysfunction, epicardial coronary … Show more

“…In many patients with ischemic heart disease, microvascular dysfunction is increasingly becoming recognized as an important mechanism of angina, in addition to the more classic obstructive atherosclerosis mechanism [34]. Microvascular dysfunction is particularly prominent in patients with recurrent angina after PCI [35]. By improving endothelial function, ivabradine may contribute to maintaining coronary dilation, hence preventing microvascular dysfunction and the recurrence of angina attacks after coronary intervention.…”

“…In Puymirat et al [13] LCX and LAD are the most common vessels involved. In the present study out of 4181 patients ,9366 lesions were stented with BMS , stainless steel BMS was used in [15,16] In the present study 17 (0.88%) patients had repeat STEMI,11(0.56%) patients had repeat NSTEMI. In Puymirat et al [13] study, 8% patients had non-fatal MI after coronary revascularization.…”

“…Restenosis can be asymptomatic posing athreat to timely interventionor can be symptomatic with a long list of noncardiac etiology to exclude (2)(3)(4). In both the cases GSMPI is recommended as a tool for detection (5,6) by virtue of its higher accuracy in comparison to exercise electrocardiogram (7,8). GSMPI has been an associate to management of ischemic heart disease (IHD) in Bangladesh for past 20 years (9).…”
