Recurrences 1 to 10 Years after Highly Selective Vagotomy in Prepyloric and Duodenal Ulcer Disease

Abstract: Three hundred and six (94%) of all consecutive patients with prepyloric or duodenal ulcer disease undergoing highly selective vagotomy at a District General Hospital were followed up after 1 to 10 completed years of observation. The 5- and 10-year cumulative recurrence rates were 11.6% and 16.8%, respectively. These figures were not related to age, sex, duration of ulcer disease, or preoperative peak acid output. Prepyloric ulcers had a significantly higher recurrence rate than duodenal ulcers during the first… Show more

“…In the treatment of duodenal ulcer disease proximal gastric vagotomy (PGV) is burdened with recurrence rates of about 15% within 5 years according to prospec tive controlled studies [1,2], Up to now a preoperative identification of patients with increased risk for postop erative ulcer recurrence is not possible [1,[3][4][5][6].…”

“…Vagotomy still remains a standard treatment for peptic ulcer, and modified operations like highly selective vagotomy have a higher rate of recurrence (Adami et al, 1984;Stoddard et al, 1984). Even before the modification of vagotomies, it was proposed that surviving branches were able to re-innervate the glandular apparatus (Dragstedt et al, 1947).…”
