Recurrence in Benign Paroxysmal Positional Vertigo: A Large, Single-Institution Study

Abstract: A large single-institution study of recurrence in BPPV is presented along with Kaplan-Meier disease-free survival curves. Female sex and history of previous BPPV were associated with increased recurrence, while previously suspected risk factors for recurrence including history of Menière's disease, diabetes, and trauma were not. Remote recurrence is more likely to involve the contralateral ear than early recurrence. These data solidify the expected course of treated BPPV allowing for improved clinical care and… Show more

“…There have been studies with conflicting results regarding the recurrence of BPPV, since multiple factors including clinical settings, and even genetic backgrounds might have influence on the occurrence of BPPV [10][11][12]17]. For example, previous studies suggested that the patients with BPPV had higher frequency of vascular comorbidities (i.e., hypertension, diabetes mellitus, and hyperlipidemia), and these might also increase the risk of the recurrence in BPPV [11,17,18].…”

“…This relationship may be related to estrogen deficiency in postmenopausal women, as estrogen may promote the development of osteoporosis and even BPPV (18). In addition, women BPPV patients have a higher risk of recurrence than men (9,31). Therefore, further research between estrogen levels and BPPV may help early diagnosis and prevention of BPPV.…”

“…Thirty articles examined 14 risk factors for recurrence of BPPV and they included 12,585 participants ( Table 2 ). Twenty-three were retrospective studies [ 1 , 2 , 3 , 4 , 6 , 7 , 8 , 10 , 11 , 12 , 13 , 16 , 18 , 19 , 20 , 21 , 23 , 24 , 25 , 26 , 29 , 30 , 31 ] and seven were prospective cohort studies [ 5 , 14 , 15 , 17 , 22 , 27 , 28 ]. In all studies, the diagnosis of BPPV was established by positive positional tests (Dix-Hallpike, roll tests).…”
