A betegségek korai felfedezésére a 19. század második felében javasoltak szűrővizsgálatokat: kialakulásukban a tudománytörténet számos jelentős személyisége, feltalálók és iparmágnások vettek részt. A szűrővizsgálatok általános követelményeit 1968-ban fogalmazták meg. A rákszűrés a daganatok másodlagos megelőzésének módszere. A széklet okkult vér kimutatásának módszerei a törvényszéki orvostanból kerültek a laboratóriumi diagnosztikába: ma a rákszűrés alapvető vizsgálata. Pozitív esetben kolonoszkópia végzése szükséges, alternatívaként a komputertomográfia, mágnesesmagrezgés-vizsgálat, kapszulaendoszkópia végezhető. Számos más módszernek ma már csak történeti jelentősége van. Az 1970-es évektől kezdődően több hazai rákszűrési kísérlet után 2018-ban indult útjára az 50-70 év közötti lakosság országos szűrése, amely remélhetőleg csökkenteni fogja a betegség kedvezőtlen morbiditási és mortalitási adatait. KULCSSZAVAK: colorectalis karcinóma, kolonoszkópia, okkult vér, rákszűrés History of colorectal carcinoma screeningThe first screening examinations for the early discovery of diseases were developed in the second half of the 19th-century, with the participation of many prominent scientific personalities, inventors and investors. The principles of screening were drafted in 1968: for colorectal cancer, they comprise the secondary prevention method. Faecal occult blood testing is the basic screening method which was adopted from forensic medicine. In positive cases, a colonoscopy is warranted. Alternatively, computer tomography, magnetic resonance imaging or capsule endoscopy can be performed. Many other methods were removed from the screening process, having only historical importance. From the 1970's, several local screening programmes were implemented in Hungary. In 2018, a new screening programme covering those aged 50 to 70 years old throughout the country was started, which will hopefully change the unfavourable morbidity and mortality of colorectal carcinoma.

In colon cancer biologic markers have been successfully utilized to follow the course of disease, to evaluate response to therapy, and to predict recurrence. With the exception of occult blood measurements, there has been little success in adopting markers to early diagnosis. The role of CEA in predicting the course of the disease is considered, as well as the utility of combining serum enzyme measurements and carcinoembryonic antigen (CEA) assays for these purposes. Aryl sulfatase B has been correlated to the stage of the disease, and enzymes involved in the metabolism of glycoproteins (sialyltransferase and glucosyltransferase) may be of importance in monitoring colon cancer. Polyamines have been observed in blood and urine of patients with a variety of cancers and their presence during therapy in increased amounts may reflect cancer cell death as well as tumor growth. Their use in diagnosis and chemo‐therapeutic monitoring in colon cancer has not yet been established. Another area of possible use of biologic markers is their measurement in colon washings. This is discussed in regard to the assay of CEA and enzymes in colonic lavage.

Colonic epithelial cells in man begin to develop some of the characteristics of malignant cells while they still appear normal on conventional morphological examination. The cells pass through specific phases during which they express increasingly abnormal proliferative characteristics. As a Phase 1 proliferative lesion develops, colonic epithelial cells do not repress DNA synthesis during their maturation, and begin to develop an enhanced ability to proliferate. The over‐all kinetics of cell proliferation remain normal, without a net retention or accumulation of cells in the mucosa. As a Phase 2 proliferative lesion forms, the cells begin to develop properties that enable them to be retained in the mucosa in increasing numbers. The over‐all kinetics of cell proliferation become abnormal, and net retention and accumulation of cells begin to be observed. These changes are accompanied by differentiation‐specific molecular errors, resulting in the abnormal persistence of metabolic pathways leading to enhanced DNA synthesis. These proliferative cellular lesions arise in hereditary familial polyposis in man, in individuals in the general population who develop isolated colonic neoplasms, and in rodents after a chemical carcinogen is given, suggesting that they are major steps on a final common molecular pathway leading to malignant transformation. Their expression is accelerated in familial polyposis. The proliferative lesions and increasingly severe degrees of cellular atypia can be screened in familial polyposis, the utility of the screening procedure is being studied. The molecular lesions leading to persistent DNA synthesis, the interaction of suspected carcinogens leading to the appearance of the above changes in model systems, and the development of more effective chemotherapy and immunotherapy against the reactions leading to enhanced DNA synthesis are topics of importance at the present time.
