Objective Tacrolimus intrapatient variability (Tac IPV) has been considered a marker for post-graft risk. We investigated pre-transplant psychometric testing to predict Tac IPV after living kidney transplantation.Methods Minnesota Multiphasic Personality Inventory-2 (MMPI-2) examined during pre-transplant evaluation by 102 recipients were analyzed. Subjects were divided into two groups, low IPV (L-IPV) and high IPV (H-IPV), by cutoffs of Tac IPV: median of 24 and value of 30. T-scores of MMPI-2 scales were used to analyze difference between L-IPV and H-IPV using independent t-tests. Stepwise multiple logistic regression was used to test whether MMPI-2 scales affected Tac IPV. Confusion matrix of logistic regression was used to explain statistical power. Cutoff values of significant scales for H-IPV were analyzed by constructing receiver operating characteristic curves.Results Hysteria (Hy) and depression (D) scale scores and Tac IPV were associated in IPV 24 (odds ratio [OR]: 1.08, p<0.01 for Hy; OR: 0.93, p<0.01 for D) and IPV 30 models (OR: 1.09, p<0.01 for Hy; OR: 0.92, p<0.01 for D). Paranoia (Pa) scale scores were associated with Tac IPV in IPV 24 model (OR=1.10, p<0.01) and were significantly higher in H-IPV 24 (p<0.01). F1 scores of confusion matrix in IPV 24 and 30 models were 0.70 and 0.71, respectively. Cutoffs of Hy, D, and Pa scales were 51, 57, and 47, respectively.Conclusion MMPI-2 profile is suggested as a predictor for high Tac IPV after living kidney transplantation.

Background The previous psychosocial evaluation of the potential living kidney donors (PLKD) requires a detailed understanding of the psychosocial benefits and the possible damages of the act of donation. Objective The aim was to create clusters by using the clinical patterns of personality and to evaluate their influence on psychopathological variables. Methods Observational, analytical and cross-sectional study that included the PLKD from February 2009 to March 2017. The patients were referred to the Hospital Psychology Unit by the Transplant Coordination Unit. The total sample was composed of 100 participants. The socio-demographic characteristics, the relationship with the recipient and the Millon Clinical Multiaxial Inventory were included. Results The final sample was composed by 100 PLKD. The mean age of the participants was 45.70, and most were women (70%). The analysis showed a final result of 3 personality clusters that best represented the data, in agreement with the DSM-5 classification. The PLKD from cluster 3 obtained greater scores in all the clinical syndromes. Conclusions The personality evaluation of the PLKD could help with the planning of monitoring protocols of the participants who were classified to cluster 3, in order to improve their post-transplant psychosocial adjustment. This result makes us consider the usefulness of the psychosocial evaluation to preserve the psychological health of the PLKD.

ResumenEl presente artículo buscó determinar la aplicabilidad de la prueba de personalidad Inventario Multifásico de Personalidad de Minnesota (MMPI) a través de los últimos 25 años, por medio de la metodología de revisión sistemática de la literatura de 41 estudios en bases de datos de libre acceso como: Organización de las Naciones Unidas para la Alimentación y la Cultura, MedlinePlus, Open Journal Systems, Dialnet, Scielo, Wiley Open Access, UNESCO, Latindex, Ebsco Host, Scielo y Redalyc; que permitieron recopilar información sobre el instrumento y su aplicación en diferentes contextos y momentos del tiempo, desde el año 1990 hasta el 2015. Se concluye que en la mayoría de las investigaciones se ha demostrado la validez y confiabilidad del MMPI y se reconoce la necesidad de continuar estudios en relación a la aplicación, validación y sistematización del instrumento. It is concluded that most of the investigations have shown the validity and reliability of the MMPI as well as the need to continue studies in relation to the implementation, validation and systematization of the instrument.
