Associação de pexias intra-abdominais e autoenxerto pediculado de túnica vaginal para tratamento da hérnia perineal em cão -relato de casoAssociation of intra-abdominal pexias and pediculated autograft of vaginal tunic for treatment of perineal hernia in dog. -case report A B S T R A C TPerineal hernia is a common pathology in elderly dogs, characterized by the caudal displacement of abdominal and pelvic structures into the perineum. The aim of this study was to evaluate the viability and applicability of the association of the techniques of colopexy, deferenopexy and pediculated autograft of tunica vaginalis for occlusion of the hernial ring via abdominal access in a geriatric dog with unilateral, uncomplicated right perineal hernia. The surgical procedure consisted of three surgical times, being the first a closed orchiectomy, followed by an autograft by abdominal and then, in a third surgical time, the deferenopexy and colopexy were performed. The animal was evaluated during a period of 360 days after surgery through clinical and ultrasound evaluations without evidence of complications or recurrence. In view of the results, it was concluded that the association of surgical techniques was effective in the treatment of perineal hernia in a geriatric dog.1 . Universidade Paraense R E S U M OA hérnia perineal é uma patologia comum em cães idosos, caracterizada pelo deslocamento caudal de estruturas abdominais e pélvicas para o períneo. Neste estudo objetivou-se avaliar a viabilidade e aplicabilidade da associação das técnicas de colopexia, deferentopexia e auto enxerto pediculado de túnica vaginal para a oclusão do anel herniário via acesso abdominal em um cão geriatra portador de hérnia perineal unilateral direita, não complicada. O procedimento cirúrgico consistiu de três tempos cirúrgicos, sendo o primeiro a realização de orquiectomia fechada, seguido do autoenxerto pediculado via abdominal e posteriormente, em um terceiro tempo cirúrgico, realizadas a deferentopexia e colopexia. O animal foi avaliado durante período de 360 dias após cirurgia mediante avaliações clínicas e ultrassonográficas sem evidencia de complicações ou recidiva. Diante dos resultados obtidos, concluiu-se que, a associação das técnicas cirúrgicas foi eficaz no tratamento da hérnia perineal em cão geriatra.

In an effort to reduce shelter intake, Miami-Dade Animal Services (MDAS) Pet Retention Program offers caretakers with an opportunity to have animals treated for certain medical conditions, free of charge. Discovery of new, simple surgical techniques for low-cost procedures provides veterinarians with more surgical solutions, expands the capacity for services provided through low-cost veterinary clinics and other shelter programs, and provides shelters with life-saving alternatives that will increase adoptability of homeless pets and reduce euthanasia rates. The aim of this clinical trial was to describe and to evaluate the use of the autologous tunica vaginalis communis as a free graft to repair perineal hernia (PH) in intact male dogs at an animal shelter facility. In 2018, seven male intact dogs, diagnosed with perineal hernias (PH), were presented to MDAS. All dogs had the surgical reconstruction of the pelvic diaphragm repaired by using the tunica vaginalis communis obtained at the time of castration, prior to the perineal access, and sutured directly into the perineal defect. Clinical outcome including postoperative complications and hernia recurrence were obtained via telephone communication. The median follow-up time was 13 months. None of the dogs included in this study had recurrence of the PH. All dogs were adopted, were transferred to animal rescue organizations or were returned to their owners after the surgical procedure. The use of tunica vaginalis communis autograft is a simple, low-cost surgical technique that requires less surgical expertise and training and can be used for perineal herniorrhaphy in dogs, without long-term recurrence.

ABSTRACT. Perineal hernia in dogs is very problematic and mostly requires surgical reconstruction of the weak pelvic diaphragm. Tissue or synthetic grafts have been used for the correction after failure of the conventional herniorrhaphy. Aim of this clinical trial was to assess the possible use of the autologous tunica vaginalis communis as a free graft for perineal hernia repair in intact male dogs. Seven unilateral and 2 bilateral perineal hernias in nine intact male dogs free from testicular and scrotal neoplasms were included in this study. The median surgical time for unilateral herniorrhaphy was 75 min. The median follow-up time was 13 months. The success of the autografting, based on no recurrence and comfort of the animals during urination and defecation, was found in ten of 11 hernias; giving a success rate of 90.91%. One hernia (9.09%) recurred 10 days after surgery. Histopathological examination of the apposing area between the graft and the adjacent tissue, taken during the repair of the recurred case at day 20, revealed neovascularization and connective tissue ingrowth. In conclusion, the tunica vaginalis autograft can be used for perineal herniorrhaphy in intact male dogs.
