Reconstruction of the superior vena cava: Benefits of postoperative surveillance and secondary endovascular interventions

Abstract: Long-term secondary patency rates justify SVC grafting for benign disease. Postoperative surveillance with contrast venography is indicated in the first year to detect graft problems. Endovascular techniques may salvage and improve the patency of SVC grafts.

“…The most common complaint is a feeling of fullness in the head and neck, exacerbated when bending over or lying flat in bed. Physical signs of SVCS may include enlarged chest wall venous collaterals, facial and neck swelling, facial cyanosis, arm swelling and pleural effusion [1,10,14,19]. Although the majority of SVCS are caused by malignant tumours, thrombosis of the SVC secondary to central venous catheter or pacemaker placement could potentially be the cause [9,13].…”

“…Before the widespread introduction of endovascular techniques, the surgical reconstruction of the SVC was the treatment of choice for bSVC syndrome even if it has been aggravated by a high rate of complications, also in highly specialized centers [14].…”
