Reconstruction of Large Iliac Crest Defects After Graft Harvest Using Autogenous Rib Graft

Abstract: Rib graft reconstruction provides a cheap and effective alternative for iliac crest reconstruction. Patients undergoing thoracotomy or thoraco-phrenico-lumbotomy for spinal reconstruction, the unutilized rib graft should be used to reconstruct the iliac defect. Reduced donor site morbidity and better cosmesis are the major benefits of reconstruction.

“…Anterior ICBG volume has been reported to range from an average of 5.3-72 cm 3 [5,8,27]. 3 ), but in a much younger study population than Belthur et al [4,41]. Results in the literature show that the amount of intramedullary bone graft that can be harvested is comparable to the amount of ICBG that can be harvested.…”

“…Possible harvesting sites such as the distal radius, olecranon, and rib are described in the literature, but they are not used very often in long bone treatment because of the small amount of graft obtained and the possible complications [3,5,11,22,35]. Autogenous iliac crest bone graft (ICBG) harvest is associated with signifi cant donor site complications with rates as high as 10% minor and 8.6% major complications.…”

“…Bapat et al [20] and Dave et al [21] used rib autograft for reconstruction of the iliac crest during surgery of thoracic spine. The rib, which was removed at the time of 16 40-70 mm 94 % Median VAS of 3 (0-9) 87.5 % "very satisfied" 0 Tutobone, Xenograft n/a not applicable or not available, VAS visual analogue scale thoracotomy, was used and fixed to the iliac crest with a press-fit technique after fashioning it from both ends.…”

“…All of the above graft materials have been used in order to minimize donor-site pain, to promote bone healing [10,11,16,17], and to improve cosmesis [7-9, 18, 19] and functional outcome [12,[20][21][22].…”

“…Reossification of the skull may contribute to this phenomenon, and in fact some authors have described a role for reconstituting the iliac crest with autologous rib following large iliac crest graft harvests. 2 The most common complication of iliac crest graft harvesting is donor site pain. Most of these data have come from the adult literature.…”
