Objective Since the beginning of the COVID‐19 epidemic, a large number of guidelines on diagnosis and treatment of COVID‐19 have been developed, but the quality of those guidelines and the consistency of recommendations are unclear. The objective of this study is to evaluate the quality of the diagnosis and treatment guidelines on COVID‐19 and analyze the consistency of the recommendations of these guidelines. Methods We searched for guidelines on diagnosis and/or treatment of COVID‐19 through PubMed, CBM, CNKI, and WanFang Data, from January 1, 2020 to August 31, 2020. In addition, we also searched official websites of the US CDC, European CDC and WHO, and some guideline collection databases. We included diagnosis and/or treatment guidelines for COVID‐19, including rapid advice guidelines and interim guidelines. Two trained researchers independently extracted data and four trained researchers evaluated the quality of the guidelines using the AGREE II instruments. We extracted information on the basic characteristics of the guidelines, guideline development process, and the recommendations. We described the consistency of the direction of recommendations for treatment and diagnosis of COVID‐19 across the included guidelines. Results A total of 37 guidelines were included. Most included guidelines were assessed as low quality, with only one of the six domains of AGREE II (clarity of presentation) having a mean score above 50%. The mean scores of three domains (stakeholder involvement, the rigor of development and applicability) were all below 30%. The recommendations on diagnosis and treatment were to some extent consistent between the included guidelines. Computed tomography (CT), X‐rays, lung ultrasound, RT‐PCR, and routine blood tests were the most commonly recommended methods for COVID‐19 diagnosis. Thirty guidelines were on the treatment of COVID‐19. The recommended forms of treatment included supportive care, antiviral therapy, glucocorticoid therapy, antibiotics, immunoglobulin, extracorporeal membrane oxygenation (ECMO), convalescent plasma, and psychotherapy. Conclusions The methodological quality of currently available diagnosis and treatment guidelines for COVID‐19 is low. The diagnosis and treatment recommendations between the included guidelines are highly consistent. The main diagnostic methods for COVID‐19 are RT‐PCR and CT, with ultrasound as a potential diagnostic tool. As there is no effective treatment against COVID‐19 yet, supportive therapy is at the moment the most important treatment option.

por Coronavirus 2019 (COVID-19), causada por el virus SARS-CoV-2, fue declarada por la Organización Mundial de la Salud (OMS) como una urgencia de salud pública de importancia global y catalogada como pandemia el 30 de enero y 11 de marzo de 2020, respectivamente (1,2). COVID-19 tiene una presentación clínica variable, desde un estado asintomático hasta un Síndrome de Dificultad Respiratoria Aguda (SDRA) y muerte (1,3,4). Las manifestaciones clínicas típicas son la presencia de fiebre (72,8%), tos (68,9%) y disnea (47,4%); sin embargo, esta dependerá del curso y la severidad de la enfermedad (1,5). Dentro de los principales factores asociados a enfermedad severa se encuentra la edad avanzada, de modo que más del 30% de los pacientes hospitalizados por COVID-19 tienen al menos 70 años. Comprendiendo que la población geriátrica constituye un grupo de riesgo, el objetivo de esta carta al editor es fomentar el debate sobre aspectos de importancia en la prevención y control del brote en adultos mayores, quienes se encuentran expuestos a un mayor número de comorbilidades y presentaciones atípicas que condicionan un diagnóstico tardío, y un pronóstico adverso (1,5-7). Actualmente, el número global de adultos mayores supera los 800 millones, constituyendo el 12% de la población mundial (6) y, como si fuera poco, dada la acelerada transición demográfica, según informes de la Organización de las Naciones Unidas (ONU), se estima que para el 2050 los adultos mayores serán unos 2.000 millones, es decir, más del 20% de la población global (1,8). IMPORTANCEThe current rapid worldwide spread of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection justifies the global effort to identify effective preventive strategies and optimal medical management. While data are available for adult patients with coronavirus disease 2019 (COVID-19), limited reports have analyzed pediatric patients infected with SARS-CoV-2. OBJECTIVE To evaluate currently reported pediatric cases of SARS-CoV-2 infection.EVIDENCE REVIEW An extensive search strategy was designed to retrieve all articles published from December 1, 2019, to March 3, 2020, by combining the terms coronavirus and coronavirus infection in several electronic databases (PubMed, Cochrane Library, and CINAHL), and following the Preferred Reporting Items for Systematic Reviews and Meta-analyses guidelines. Retrospective cross-sectional and case-control studies, case series and case reports, bulletins, and national reports about the pediatric SARS-CoV-2 infection were included. The risk of bias for eligible observational studies was assessed according to the Strengthening the Reporting of Observational Studies in Epidemiology reporting guideline.FINDINGS A total of 815 articles were identified. Eighteen studies with 1065 participants (444 patients were younger than 10 years, and 553 were aged 10 to 19 years) with confirmed SARS-CoV-2 infection were included in the final analysis. All articles reflected research performed in China, except for 1 clinical case in Singapore. Children at any age were mostly reported to have mild respiratory symptoms, namely fever, dry cough, and fatigue, or were asymptomatic. Bronchial thickening and ground-glass opacities were the main radiologic features, and these findings were also reported in asymptomatic patients. Among the included articles, there was only 1 case of severe COVID-19 infection, which occurred in a 13-month-old infant. No deaths were reported in children aged 0 to 9 years. Available data about therapies were limited.CONCLUSIONS AND RELEVANCE To our knowledge, this is the first systematic review that assesses and summarizes clinical features and management of children with SARS-CoV-2 infection. The rapid spread of COVID-19 across the globe and the lack of European and US data on pediatric patients require further epidemiologic and clinical studies to identify possible preventive and therapeutic strategies.
