Recommendations for the nomenclature of cognitive change associated with anaesthesia and surgery—2018

Abstract: Cognitive change affecting patients after anaesthesia and surgery has been recognised for more than 100 yr. Research into cognitive change after anaesthesia and surgery accelerated in the 1980s when multiple studies utilised detailed neuropsychological testing for assessment of cognitive change after cardiac surgery. This body of work consistently documented decline in cognitive function in elderly patients after anaesthesia and surgery, and cognitive changes have been identified up to 7.5 yr afterwards. Impor… Show more

“…Nomenclature that incorporates POD with more persistent cognitive problems coincident with surgery and general anaesthesia has been proposed. 50 Although our study is not large enough to draw definitive conclusions regarding pharmacological choices for particular surgeries that might influence PACU-D, some interesting insight can be developed. The univariable analysis revealed that propofol infusion was associated with increased odds of developing PACU-D. Interestingly, TIVA alone did not exhibit the same association.…”

“…In the new recommendations for the nomenclature of cognitive change associated with anesthesia and surgery-2018, however, it is required that definitions of POD must meet the diagnostic criteria of delirium in the Diagnostic and Statistical Manual of Mental Disorders fifth edition. 2 Evidently, the use of Intensive Care Delirium Screening Checklist ≥3 points for definitions of POD in this study could not achieve the diagnostic criteria of the Diagnostic and Statistical Manual of Mental Disorders fifth edition for delirium. Actually, the intensive care unit delirium assessment tools are not adequately sensitive to identify POD in non-intensive care unit settings.…”

“…An adverse outcome after surgery considered particularly relevant in (but not restricted to) the elderly is postoperative cognitive decline 5 . This deterioration in cognitive functioning following surgery has been termed as postoperative neurocognitive disorder (NCD) recently 6 …”

“…5 This deterioration in cognitive functioning following surgery has been termed as postoperative neurocognitive disorder (NCD) recently. 6 Although it has been reported that postoperative NCD is a multifactorial phenomenon, there are few well-established risk factors. 7 Accumulating evidence suggests that postoperative NCD might result from increased inflammatory activity.…”
