Recent developments of nanotherapeutics for targeted and long-acting, combination HIV chemotherapy

Abstract: Combination antiretroviral therapy (cART) given orally has transformed HIV from a terminal illness to a manageable chronic disease. Yet despite the recent development of newer and more potent drugs for cART and suppression of virus in blood to undetectable levels, residual virus remains in tissues. Upon stopping cART, virus rebounds and progresses to AIDS. Current oral cART regimens have several drawbacks including (1) challenges in patient adherence due to pill fatigue or side-effects, (2) the requirement of … Show more

“…However, the physicochemical and biologic properties of the nanotherapeutics can be modulated by the addition of either polyanionic, hydrophobic or hydrophilic side chains on the scaffold. Moreover, the compound can be further modified by the addition of targeting ligands [49]. Indeed, the construction design of the lipid or polymer scaffold is virtually unlimited, but some practical considerations must be taken into account.…”

“…Indeed, the construction design of the lipid or polymer scaffold is virtually unlimited, but some practical considerations must be taken into account. For example, in many current spherical non-covalent nanoparticles of physiological relevant sizes, the pharmaceutical active drugs comprise only 1% of the total molecular weight [49].…”

“…Even though the sustained ART-free remission is a well-accepted and effective strategy to control HIV infection, the main focus of HIV research is to develop strategies that prolong protection and target latent HIV+ cells. Recent studies have demonstrated designing a long-acting (LA) ARV delivery system would boost the HIV prevention and treatment strategy (69). The LA ARV delivery system has shown to enhance drug-solubility, stability, biodistribution, pharmacokinetics, efficiency, and concurrent drug safety, due to reduced ARV associated side-effects, such as mitochondrial toxicity, renal abnormality, and reduced bone mineral density (10–12).…”

“…Its purpose is to increase the duration of the release, achieve a different mechanism, or delay the initial release [6]. Polymeric nanoparticles have been cited as one kind of nanomedicine that can be advantageous for the treatment of HIV [7,8].…”
