Recent Developments in Inhaled Triazoles Against Invasive Pulmonary Aspergillosis

Abstract: Invasive pulmonary aspergillosis (IPA) presents high incidence and high mortality in immunocompromised patients. To combat or prevent IPA, triazoles such as voriconazole or itraconazole and posaconazole are becoming the first-or second-line therapy, respectively. However, triazoles present issues of oral bioavailability, high liver metabolism and/or drugdrug interactions, increasing the variability of systemic concentrations. To overcome these issues, inhalation is a promising route for delivering triazoles for… Show more

“…14 Nanoparticle formulations are used in many therapeutic fields, especially inhalation drug delivery systems, which ensure drug release after pulmonary administration. 15 Nano formulations have been increasingly proposed for lung drug delivery applications of antibacterial, 16 antifungal, 17 and antiviral drugs, 18 which can be made to achieve appropriate morphological and aerodynamic characteristics, in the form of dry powders for pulmonary inhalation. 19 Solid lipid nanoparticles (SLNs) are lipophilic nanoscale aqueous suspension and have the ability to incorporate drugs as a nanosystem, SLNs have evolved as a drug delivery system that can be potentially used for pulmonary drug targeting.…”

“…Aspergillosis is mostly caused by 1 of the 4 following species: Aspergillus fumigatus, Aspergillus flavus, Aspergillus niger, and Aspergillus terreus. 2 Once the spores of Aspergillus are inhaled, they may germinate into hyphae and invade into the lung tissue of immunocompromised subjects, causing pulmonary lesions, dissemination to other organs, and potential mortality. Native diseases such as autosomal dominant hyper-IgE syndrome associated with STAT-3 mutation (Job syndrome) also predispose patients to recurrent sinopulmonary infections by bacteria and opportunistic fungi.…”
