Reasons for Failure to Diagnose Colorectal Carcinoma at Colonoscopy

Abstract: Colonoscopy missed 17 of 286 cancers (5.9 %). The reasons why cancers were missed relate to incomplete colonoscopy, poor bowel preparation, misinterpretation of what was seen, failure to carry out adequate biopsy (and follow-up) of lesions seen, and systems failures related to follow-up investigations in patients who had an incomplete colonoscopy. The fact that colonoscopy and barium enema investigations may fail to diagnose cancers has important medicolegal implications. The recognition that colonoscopy may m… Show more

“…Failure to routinely intubate the cecum is in fact one of the reasons that limit colonoscopy's effectiveness; a study from Japan and another from New Zealand found that interval cancers were due to failure to reach the cecum, in 27% and 54% of cases respectively 33,34 . .…”

“…Numerous studies demonstrate that a significant proportion of patients with negative initial investigations have a bleeding source on repeat OGD in 35%-75% or repeat colonoscopy in 6% of cases [45,[71][72][73][74][75][76] . Thus a re-look endoscopy may be recommended as a cost-effective first step before further evaluation [7] .…”

“…Repeating a colonoscopy has a slightly lower yield, but it is a reasonable and necessary option if the previous colonoscopy was suboptimal because of incomplete or poor preparation. It has been shown that colonoscopy can fail in the diagnosis in 5% of colorectal tumors due to several reasons: an incomplete exploration, poor bowel preparation, misinterpretation of findings, inadequate biopsies of lesions [27] , or just not seeing the lesion.…”
