The mouth prevalence of tooth loss was 47.7%, and mean (SD) number of missing teeth was 5.9 (4.7) teeth. Age, level of education, monthly income and physical impairment were associated with number of missing teeth (P˂.05). Speech difficulty, appearance problem, chewing difficulty, feeling of bereavement, mobility of adjacent teeth, halitosis and premature ageing were associated with increasing number of missing teeth (P˂.05).

In the dental implant era, why do we still bother saving teeth?

Teeth are vital sensory organs that contribute to our daily activities of living. Unfortunately, teeth can be lost for several reasons including trauma, caries, and periodontal disease. Although dental trauma injuries and caries are more frequently encountered in a younger population, tooth loss because of periodontal disease occurs in the older population. In the dental implant era, the trend sometimes seems to be to extract compromised teeth and replace them with dental implants. However, the long‐term prognosis of teeth might not be comparable with the prognosis of dental implants. Complications, failures, and diseases such as peri‐implantitis are not uncommon, and, despite popular belief, implants are not 99% successful. Other treatment options that aim to save compromised or diseased teeth such as endodontic treatment, periodontal treatment, intentional replantation, and autotransplantation should be considered on an individual basis. These treatments have competing success rates to dental implants but, more importantly, retain the natural tooth in the dentition for a longer period of time. These options are important to discuss in detail during treatment planning with patients in order to clarify any misconceptions about teeth and dental implants. In the event a tooth does have to be extracted, procedures such as decoronation and orthodontic extrusion might be useful to preserve hard and soft tissues for future dental implant placement. Regardless of the treatment modality, it is critical that strict maintenance and follow‐up protocols are implemented and that treatment planning is ethically responsible and evidence based.

Level of difficulty of tooth extractions among roughly 100,000 procedures in primary care

Objectives The study examined treatment codes of extracted teeth and aimed to assess degree of difficulty concerning all tooth extractions. Materials and methods Retrospective data on treatment codes of all tooth extractions during a two-year period were obtained from the patient register in primary oral healthcare of the City of Helsinki, Finland. Prevalence, indication, and method of extraction appeared in the treatment codes (EBA-codes). Degree of difficulty was determined from the method and classified as non-operative or operative and as routine or demanding. Statistics included frequencies, percentages, and χ2 test. Results Total number of extraction procedures was 97,276, including 121,342 extracted teeth. The most frequent procedure was a routine extraction of a tooth with forceps (55%, n = 53,642). The main reason for extraction was caries (27%, n = 20,889). Of the extractions, 79% (n = 76,435) were non-operative, 13% (n = 12,819) operative, and 8% (n = 8,022) multiple extractions in one visit. Level of difficulty was distributed as routine non-operative (63%), demanding non-operative (15%), routine operative (12%), demanding operative (2%), and multiple extractions (8%). Conclusions Two-thirds of all tooth extractions in primary care were relatively simple. However, 29% of procedures were classified as demanding. Clinical relevance As earlier methods for assessing level of difficulty were aimed at third molars alone, an analysis was presented for all tooth extractions. This approach may be useful for research purposes, and the profile of tooth extractions and their difficulty level may be practical also for decision-makers in primary care.
