Objectives: To explore users' experiences of a tailored, interactive web application that supports behaviour change in stress management and to identify if and in that case what in the web-based programme that needed further development or adjustment to be feasible in a randomised controlled trial. Method: The design of this study was explorative with a qualitative approach. Nine individuals were recruited among the staff at a university. Semi-structured individual interviews were conducted and analysed using qualitative content analysis, after the participants had completed the web-based stress management programme. Results: One theme was identified, Struggling with what I need when stress management is about me, describing the paradox in having a programme that is perceived as supporting stress management while also being perceived as extensive and time consuming. The theme was divided in two categories: Defining the needs, where the users expressed what they needed from the programme and their everyday environment, to be able to use the programme, and It is about me, where the programme was described as helping the users understand their own stress. Conclusion: The participants expressed acceptance of using a web-based programme for stress related problems. The perceived extensiveness of the programme must be considered in further development.

BackgroundChronic low back pain is a common chronic condition whose treatment success can be improved by active involvement of patients. Patient involvement can be fostered by web-based applications combining health information with decision support or behaviour change support. These so-called Interactive Health Communication Applications (IHCAs) can reach great numbers of patients at low financial cost and provide information and support at the time, place and learning speed patients prefer. However, high attrition often seems to decrease the effects of web-based interventions. Tailoring content and tone of IHCAs to the individual patient ´s needs might improve usage and therefore effectiveness. This study aims to evaluate a tailored IHCA for people with chronic low back pain combining health information with decision support and behaviour change support.Methods/DesignThe tailored IHCA will be tested regarding effectiveness and usage against a standard website with identical content in a single-blinded randomized trial with a parallel design. The IHCA contains information on chronic low back pain and its treatment options including health behaviour change recommendations. In the intervention group the content is delivered in dialogue form, tailored to relevant patient characteristics (health literacy, coping style). In the control group there is no tailoring, a standard web-page is used for presenting the content. Participants are unaware of group assignment. Eligibility criteria are age ≥ 18 years , self- reported chronic low back pain, and Internet access. To detect the expected small effect (Cohen’s d = 0.2), the sample aims to include 414 patients, with assessments at baseline, directly after the first on-page visit, and at 3-month follow-up using online self-report questionnaires. It is expected that the tailored IHCA has larger effects on knowledge and patient empowerment (primary outcomes) compared to a standard website. Secondary outcomes are website usage, preparation for decision making, and decisional conflict.DiscussionIHCAs can be a suitable way to promote knowledge about chronic low back pain and self-management competencies. Results of the study can increase the knowledge on how to develop IHCAs which are more useful and effective for people suffering from chronic low back pain.Trial registrationInternational Clinical Trials Registry DRKS00003322

Limited access to pain management programs was identified as one of the hindering factors in pain treatment. Several internet-based interventions have been developed to improve accessibility. A systematic review of trials of such programs in 2010 documented a preliminary promising effect. A PubMed electronic search was used to identify a systematic randomized controlled trail (RCTs) published 1 January, 2010 to 4 November, 2014 that examined the effectiveness of internet-based pain management programs. The methodological vigorousness of trials was assessed by Jadad scoring system. Out of the 20 RCTs, 5 were on chronic pain, 2 were on acute non-specific pain, while 13 were on disease related pain (rheumatological and neurological diseases, burns, post-operative and cancerrelated). Most studies had moderate methodological quality and showed consistent results with respect to effectiveness of internet based programs in reduction of pain, improvement in functionality and psychological well-being. Whilst the current systematic review found a significant pain reduction attributed to internet-based pain interventions further, high-quality RCT are needed to confirm such promising findings.
