Realist complex intervention science: Applying realist principles across all phases of the Medical Research Council framework for developing and evaluating complex interventions

Abstract: The integration of realist evaluation principles within randomised controlled trials (‘realist RCTs’) enables evaluations of complex interventions to answer questions about what works, for whom and under what circumstances. This allows evaluators to better develop and refine mid-level programme theories. However, this is only one phase in the process of developing and evaluating complex interventions. We describe and exemplify how social scientists can integrate realist principles across all phases of the Medi… Show more

“…This fits with recent calls for realist feasibility and pilot studies. 200 Implications for the implementation of robot-assisted surgery Whereas phase 2 focused on the testing of theories related to communication, teamwork and decision-making, phase 1 captured a significant number of data regarding the different ways in which RAS was introduced across nine NHS hospitals, and stakeholders' perspectives on the value of those approaches and the contexts in which they are effective. While acknowledging that the theories regarding implementation remain to be empirically tested, our findings suggest that, for health-care organisations seeking to introduce RAS or to increase the use of an already purchased da Vinci robot, the following strategies may be beneficial:…”

“…Advocates of a realist approach have suggested that while the influential Medical Research Council (MRC) guidance on the evaluation of complex interventions (Craig et al, 2008) discusses ways in which evaluation may be used to build theory and understand causal mechanisms, it makes insufficient mention of the role of context for implementation (Fletcher et al, 2016). It has separately been argued that commonly applied definitions of 'complexity' may not fully engage with understandings of the concept from a systems perspective (Rickles, Hawe, & Shiell, 2007), and that such engagement could be useful in supporting effective implementation and evaluation (May, Johnson, & Finch, 2016;Pawson, 2013).…”

“…Application of realist evaluation principles 8,9 may help guide scientific inquiry in this regard: supporting researchers to answer developmental-stage questions concerning the contextual factors that support positive outcomes, and the theoretical processes leading to change. Such a line of inquiry will help lay the groundwork necessary for future rigorous research on this highly complex form of intervention.…”
