Real‐time intraoperative ultrasound biomicroscopy for determining graft orientation during Descemet's membrane endothelial keratoplasty

Abstract: Purpose To evaluate the intraoperative use of ultrasound biomicroscopy (UBM) during Descemet's membrane endothelial keratoplasty (DMEK) to determine graft's orientation. Methods Prospective interventional study of eight eyes of seven patients who underwent DMEK. Following the identification of correct graft orientation using 'Blue cannula tip' sign during DMEK surgery, UBM was used to ascertain graft's orientation. The rate of successful DMEK graft orientation determined by the UBM was registered and verified … Show more

“…132 , 133 One of the most challenging aspects of this procedure is orientating the graft so that the endothelium is facing down into the AC. Several techniques have been developed to aid graft orientation, eg, intraoperative OCT, 134 ultrasound biomicroscopy, 135 and stamping, staining or tearing one side of the graft before insertion. 136 The indications for DMEK are similar, but not identical, to those of DSAEK.…”

“…Given the growing popularity of intraoperative AS-OCT, and the pattern of parallel advancements of these two technologies, intraoperative UBM is a logical future step for diagnostic/therapeutic UBM applications in pediatric eye disease. Intraoperative UBM has been reported in adult subjects [98] and in a single pediatric case report [99], but the full potential of UBM intraoperatively has probably not yet been realized.…”
