Real-time elastography in Crohn’s disease: feasibility in daily clinical practice

Abstract: Ultrasound (US) is the first-line investigation in patients with abdominal symptoms and it has a role in the diagnosis and monitoring of inflammatory bowel diseases. Strain elastography (SE) is a US method for estimating tissue elasticity. This method is still not routinely used in clinical practice, although it is suggested by the EFSUMB Guidelines for the characterization of intestinal stenosis in Crohn's disease. The purpose of this Pictorial Essay is to confirm the feasibility of elastography in Crohn's di… Show more

“…After duplicates were removed, 31 studies (26 full text articles [8, 11 -14, 16, 20 -39] and 5 abstracts [40 -44] were assessed for eligibility. Of the 31 records selected, 2 [20,29] were excluded because they used other semi-quantitative or color scales as a parameter of bowel stiffness, 1 reported the results in kPa [44], 11 [8,11,12,16,23,24,27,28,31,33,34] were review articles, 3 [21,22,32] were excluded as they used magnetic resonance as a reference standard, and 8 [13, 30, 35 -38, 40, 43] for other reasons (▶ Fig. 1).…”

“…For SE examinations, it was necessary to use slight pressure and hold the probe still for 5 seconds, with a quality indication above 60 to ensure high‐quality elastic images. If the region of interest (ROI) was large and included the bowel lumen and sufficient amounts of adjacent bowel wall, we then traced the outline of the intestinal wall and lumen with yellow lines.…”

“…Moreover, comparing the SE results with histologic features, the strain values correlate with the histologic characteristics of fibrosis, such as collagen and muscular fibers deposits [101,102]. The assessment of intestinal fibrosis and the differentiation of inflammation from fibrosis have been studied using a semi quantitative visual color scale and the strain ratio (SR) which is the ratio between the mean strain in the normal intestinal tissue near the lesions and the mean strain of the affected intestinal tissue [106]. SR demonstrated a good ability to diagnose severe intestinal fibrosis, but failed to differentiate the degree of inflammation [102].…”
