Reactive Epstein-BarrVirus--Related Polyclonal Lymphoproliferative Disorder in a Patient with AIDS

Abstract: Reactivation of Epstein-Barr virus infection and polyclonal expansion of lymphocytes is well-documented in solid organ and hematopoietic stem cell transplant recipients and is considered a potential precursor to lymphoma. We report an analogous case of posttransplantation polyclonal lymphoproliferative disorder in a patient with human immunodeficiency virus infection who was successfully treated with antiretroviral therapy.

“…Most lesions are EBV þ . 89,[161][162][163][164] Many cases are monoclonal, based on genetic studies, but structural alterations in oncogenes and tumor suppressor genes are generally rare. 163 Although clinical outcome information is limited, at least one patient experienced regression of their polymorphic lymphoid lesion following antiviral therapy.…”

Although most lymphomas that arise in patients infected with HIV are diffuse aggressive B-cell lesions, other lesions, which are ''benign'' lymphoid proliferations, may be associated with significant clinical consequences.…”

“…Burkitt lymphoma, diffuse large B-cell lymphoma and Hodgkin lymphoma [1,3]. This case represents an EBV-associated polymorphic lymphoid proliferation resembling those more often encountered in transplant recipients [1,[4][5][6][7][8]. Such lymphoproliferations include early infectious mononucleosis (IM)-like PTLDs and polymorphic PTLDs, entities that show overlapping pathological features and represent closely related points on a continuous spectrum of disease.…”

“…However, we accept that the architectural effacement was relatively minor and that small clones can rarely be encountered in early IM-like PTLDs [2]. Although polymorphic PTLDs constitute a relatively high proportion of the lymphoid proliferations in transplant patients, they have only rarely been reported in association with HIV infection and there is relatively little information on the response to treatment and prognosis [1,2,[4][5][6][7][8].…”
