Reactivation of Herpes Zoster Keratitis in an Adult After Varicella Zoster Vaccination

Abstract: HZ keratouveitis is the result of cell-mediated immunity (CMI) directed toward viral antigens within the eye. The live attenuated HZ vaccine, Zostavax, boosts the recipient's CMI to prevent reactivation of HZ. However, patients with a history of HZ keratitis may have persistent viral antigens in their corneas and can develop recurrence of keratouveitis because of the vaccine-induced increase in CMI. Vaccination should be undertaken with caution in patients with a history of HZ ophthalmicus.

“…The increase in premalignant cell changes in non-naïve individuals, as suggested by the FDA, is consistent with the knowledge that vaccination can cause reactivation of both target and non-target viruses (6)(7)(8)(9)(10)(11)(12). For Gardasil, the HPV types 16 and 18 are called target HPVs since the vaccine contains antigens for these two HPV types.…”

“…La primera división del V par craneal se afecta 20 veces más que la segunda y tercera (4). Cuando la enfermedad afecta la primera división de este nervio, recibe el nombre de herpes zóster oftálmico y produce una disminución o pérdida de la sensibilidad corneal (5,6). El herpes zóster oftálmico se puede presentar en cualquier persona que haya padecido varicela (2).…”

“…This data projected to the United States population reveal that there may be up to 90,000 HZ eye complications annually [50]. Induction or exacerbation of VZV keratitis has been reported following Varivax or Zostivax administration in two children and one adult, leading some clinicians to speculate that persistent viral antigens in the cornea contribute to immune-mediated pathology arising from a vaccine-induced boost in cellular immunity [5153]. Three patients experienced HZ following vaccination that led to retinal necrosis, and in at least one case, the vaccine strain was identified in the vitreous of the infected eye by PCR [54,55].…”
