This is partly due to the fact that these situations are more common than one would think (Makridakis et al, 2019;Peterson et al, 2022;Schiff et al, 2005;Scott et al, 2021), as up to 35% of physicians and 42% of patients reported such errors (Berner & Graber, 2008) and are mostly preventable (Lee et al, 2022;Staal et al, 2022).
