Re-treatment after laser in situ keratomileusis for correction of myopia and myopic astigmatism

Abstract: LASIK re-treatment for residual myopia, by lifting the original flap, is an effective option. Refractive results are fairly predictable, and refraction stabilises by 3 months after re-treatment. Lifting the corneal flap after cutting the epithelium on the flap edges is easy to perform and has a very low incidence of epithelial ingrowth.

“…[2][3][4][5][6][7][8][9] Recent studies of LASIK with longterm follow-up (7 to 11 years) [10][11][12][13][14][15] report few serious complications that lead to visual deterioration. However, the rate of retreatment after LASIK remains as high as 5.5% to 27.5%, 13,14,[16][17][18][19] and most retreatments are for refractive regression, which refers to a return to a certain degree of refraction postoperatively. Moreover, studies with long-term follow-up confirm that some refractive regression occurs in most cases after LASIK.…”

“…The reported incidence after LASIK ranges from 2% to 43%. [5][6][7][8][9][10][11] The incidence is higher with older age, hyperopic corrections, and in cases in which epithelial problems were observed during the first surgery, such as undiagnosed anterior basement membrane dystrophy. [11][12][13] The incidence of epithelial ingrowth is reported to be lower with flaps created by a femtosecond laser than flaps created by a mechanical microkeratome in primary surgeries and enhancements.…”

“…Similar to LASIK, an enhancement procedure after ReLEx may be required in some cases as the refractive outcome might be dependent on various factors such as wound healing, the extent of original refractive status, patient expectations, as well as surgeon treatment philosophies and techniques [3]. To our knowledge, this is the first detailed description of enhancement after FLEx.…”

“…A bit of residual intrastromal lenticule tissue which has not been detected might still remain in the corresponding zone, which could cause irregular corneal topography, high RMS value of ocular aberration and astigmatism. The indications for enhancement of LASIK are residual myopia, hyperopia, or astigmatism greater than 0.75 D, with evidence of refractive stability [3]. According to these indications, it seemed necessary to perform an enhancement procedure on this patient to improve her visual outcomes.…”
