Rationale and Design Issues of the Randomized Intervention for Children With Vesicoureteral Reflux (RIVUR) Study

Abstract: OBJECTIVE Our goal is to determine if antimicrobial prophylaxis with trimethoprim/sulfamethoxazole prevents recurrent urinary tract infections and renal scarring in children who are found to have vesicoureteral reflux after a first or second urinary tract infection. DESIGN, PARTICIPANTS, AND METHODS The Randomized Intervention for Children With Vesicoureteral Reflux (RIVUR) study is a double-blind, randomized, placebo-controlled trial. Six hundred children aged 2 to 72 months will be recruited from both prim… Show more

“…Details on methods for patient selection, data collection, and power analysis for the RIVUR trial have been described previously (8,9). The RIVUR trial was a multicenter, randomized, placebo-controlled trial in 607 children aged 2-71 months with grade 1-4 primary VUR diagnosed after a first or second febrile or symptomatic UTI.…”

“…The rationale and trial design have been described previously. [2][3][4] Briefly, we enrolled infants and children aged 2 to 71 months with grade I through IV VUR after a first or second febrile or symptomatic UTI. Primary exclusion criteria included index UTI diagnosis .112 days before enrollment, comorbid urologic anomalies (Society of Fetal Ultrasonography grade 4 hydronephrosis with renal parenchyma atrophy, ureterocele, urethral valve, solitary kidney, profoundly small [.2 SD below the mean] kidney, multicystic dysplastic kidney, neurogenic bladder, pelvic kidney, or fused kidney), contraindications for use of trimethoprimsulfamethoxazole, and selected other medical conditions.…”

“…Although eligibility was based on local reports for ultrasound and VCUG, images were also assessed by 2 pediatric radiologists using a standardized reading protocol. 4,15 We report these data from the central radiologists. VUR was graded according to the system of the International Reflux Study Group.…”

“…The most recent study to be initiated is a North American initiative: the Randomized Intervention for Children With Vesicoureteral Reflux (RIVUR) study [18]. It is a double-blind, randomised, placebo-controlled trial, which plans to enroll 600 children, 2-72 months of age, with grades I to IV vesicoureteral reflux after the Table 1 Summary of the four prospective, randomised, controlled trials of antibiotic prophylaxis for preventing febrile urinary tract infections and pyelonephritis published to date (tot total number of randomly chosen patients in each reflux group, + number of children with recurrence of febrile UTIs)…”
