Rare side effect of adjuvant ipilimumab after surgical resection of melanoma: Guillain-Barré syndrome

Abstract: Guillain-Barré syndrome is a life-threatening neurological disorder that presents with rapid ascending paralysis and areflexia. Guillain-Barré syndrome is traditionally associated with infections from a gastrointestinal or respiratory tract source. We report the case of a 71-year-old man with melanoma who was treated with ipilimumab as adjuvant immunotherapy and subsequently developed Guillain-Barré syndrome. The diagnosis was made clinically through physical exam findings. He was successfully treated with a c… Show more

“…54.5% (18/33) of cases were consistent with AIDP [ 16 19 , 21 , 22 , 24 , 26 30 , 33 , 34 , 39 , 42 ] and axonal damage accounted for 12.1% (4/33, AMSAN: 3 and AMAN: 1) [ 15 , 23 , 25 , 27 ]. The subtypes in four cases were equivocal because they showed mixed features of axonal injury and demyelination [ 23 , 31 , 35 , 36 ]. There were also three cases, two with MFS [ 13 , 20 ] and the other with MFS-GBS overlap [ 32 ].…”

“…Slight pleocytosis (i.e., cell count > 5/ μ l) was detected in 4/33 cases (12.1%) with a maximum cell count of 15/ μ l and a median CSF protein of 115 mg/dl [ 15 , 19 , 27 , 37 ]. CSF protein values and cell counts were normal in three cases [ 23 , 25 , 35 ], and there was elevated CSF immunoglobulin G levels in one case [ 25 ].…”

“…4 5 The link between checkpoint inhibitors and the development of GBS has been described in case reports. [6][7][8][9][10][11] Conversely, there have been reports highlighting the safety of ICI administration in patients with a history of GBS. [12][13][14] This data alerts us to the importance of understanding the incidence of this type of event with the ICI class.…”

“…Guillain-Barré syndrome (GBS) is a rare neurological disorder characterised by the immune system’s misguided attack on the peripheral nerves, resulting in muscle weakness and paralysis and is the most common cause of flaccid paralysis worldwide 4 5. The link between checkpoint inhibitors and the development of GBS has been described in case reports 6–11. Conversely, there have been reports highlighting the safety of ICI administration in patients with a history of GBS 12–14…”
