Rapid Typing of Borrelia burgdorferi Sensu Lato Species in Specimens from Patients with Different Manifestations of Lyme Borreliosis

Abstract: To further investigate the pathogenic potential of different Borrelia burgdorferi genospecies, specimens from 27 patients with different manifestations of Lyme borreliosis were analyzed by PCR and reverse line blotting (RLB). In samples from Lyme arthritis patients, B. burgdorferi sensu stricto was predominantly identified, while in patients with neuroborreliosis or acrodermatitis, Borrelia garinii and Borrelia afzelii, respectively, were exclusively detected. The results demonstrate that PCR-RLB is a valuable… Show more

“…Since B. burgdorferi, the spirochete responsible for this zoonotic infection, is phenotypically and genotypically heterogeneous, it causes variability in the clinical aspects of the disease. Arthritis and carditis are preferentially associated with B. burgdorferi sensu stricto, the degenerative skin disorder acrodermatitis chronica et atrophicans is primarily associated with Borrelia afzelii, and neuroborreliosis is primarily associated with Borrelia garinii (13,18). However, B. burgdorferi sensu stricto is also responsible for human neuroborreliosis in the United States (31).…”

“…The final stage is chronic Lyme borreliosis and is usually characterized by development of reoccurring arthritis in large joints as well as a continuation of neurologic manifestations (41,47). The three Borrelia genospecies known to cause disease exhibit somewhat different manifestations; for example, B. burgdorferi tends to be associated with the development of arthritis, B. garinii tends to be associated with neuroborreliosis, and B. afzelii tends to be associated with acrodermatitis chronicum atrophicans (3,29,51).…”

“…During this time, Borrelia changes its outer proteins profi le and in the salivary glands binds immunosupresive Salp15 (6). "Armed" with this protein, Borrelia leaves tick (7) and enters to the human (8). the other hand, cp9 is probably a fragment of ancestral cp32.…”

“…Neurological complications are frequently associated with infections caused by B. garinii and B. burgdorferi 6 . Among dermatological manifestations, typical are erythema, mentioned above, and acrodermatitis chronica atrophicans, which is caused mostly by B. afzelii 7,8 . Lymphocytoma and circumscribed scleroderma are less common.…”
