Objective: To compare silicone nasal septal splints with integral airway and a polyvinyl alcohol (PVA) sponge after septoplasty in terms of patient comfort (both with the pack in place and during removal) and postoperative complications. Methods: This study involved 169 patients who underwent septoplasty without additional nasal procedures due to nasal septal deviation. They were allocated into two groups. Group A comprised 90 patients who underwent septoplasty and who were packed with silicone nasal septal splints. Group B comprised 76 patients who underwent septoplasty and who were packed with a PVA sponge. They were removed on the second postoperative day. Patients were asked to record pain levels using a visual analog scale (VAS). Results: The patients in the groups had similar mean ages: 29.77 years (range, 19-74 years) and 23.77 years (range, 21-37 years) in Group A and Group B, respectively (p>0.05). VAS scores were significantly lower in Group A than in Group B at the 1 st , 2 nd , 4 th , 8 th , 12 th , 24 th , 48 th , and 72 nd postoperative hours (p<0.0001). There were significantly higher pain levels associated with PVA sponge pack removal than with silicone nasal septum splint removal (average pain scores: 3.57 vs. 1.99, respectively; p<0.05). Conclusion: Intranasal septal splints with integral airway result in less postoperative pain during removal without increasing postoperative complications; thus, they can be used as an effective alternative to PVA sponge packing after septoplasty.

The Effects of Different Types of Nasal Packing on Odor Function and Mucociliary Function After Septum Surgery

Background: In this study, we evaluated how the Merocel and nasal splint packing placed in the nose after septoplasty surgery affects the olfactory and mucociliary functions of the nose in the early period, and compared the 2 packing with each other. Material and Method: The study included 60 patients with isolated septal deviation and 30 patients in the control group. The patients were randomly divided into 2 groups. Nasal splint was inserted after septoplasty in group A (n = 30). Merocel was inserted in group B (n = 30). The Sniffin sticks test and saccharin test were applied to the patients before surgery and 15 days after the surgery. The same tests were applied to the control group consisting of 30 patients and the results were compared. Results: No complications, such as postoperative bleeding, submucoperichondrial hematoma, or abscess formation, were found in both groups. Mucociliary function was improved after septoplasty, and it was statistically significant, but there was no statistically significant difference between both packing groups. A statistically significant difference was found for the odor test in patients who used nasal splint packing in comparison with patients who used Merocel in the early period. Conclusion: The odor test showed significant differences between the 2 groups and this was statistically significant in the early period. Mucociliary function was better after surgery, but there was no statistical difference in the different nasal packing groups.

Transseptal Suturing Reduce Patient Anxiety after Septoplasty Compared to Nasal Packing

Summary:Background: We measured postoperative anxiety in patients who underwent transseptal suturing or nasal packing after septoplasty. Materials and Methods: Transseptal suturing was performed on Group 1 patients and nasal splints with airway were placed after septoplasty in Group 2 patients. Postoperative 48-h anxiety levels of both groups were measured using the State-Trait Anxiety Inventory (STAI) clinical assessment scale, prior to removal of nasal packing in Group 2. Results: Transseptal suturing was performed after septoplasty in 28 patients and nasal packing in 34 patients. The State-Trait Anxiety Inventory clinical assessment state (STAI-S) and trait (STAI-T) instruments were used to measure postoperative anxiety. The STAI-S scores were found 35.00 in the transseptal suturing group and 43.8 in the nasal packing group; the difference was found significant (p < 0.05). The STAI-T scores were found 42.6 in the transseptal suturing group and 45.7 in the nasal packing group; the difference was not found significant (p > 0.05). The rate of minor hemorrhage was found 10.7% in Group 1 patients. Conclusions: Transseptal suturing is simple and reliable when performed after septoplasty. The technique is painless and comfortable, and reduces patient anxiety (compared to that associated with nasal packing) with only a minor increase in operating time and hemorrhage.

Rapid Rhino versus Merocel nasal packs in septal surgery

Abstract: Rapid Rhino nasal packs are less painful and cause less bleeding, compared with Merocel packs, with no side effects. Thus, their use for nasal packing after septal surgery is recommended.
