Rapid-Response Extracorporeal Membrane Oxygenation to Support Cardiopulmonary Resuscitation in Children With Cardiac Disease

Abstract: Background-Survival of children with in-hospital cardiac arrest that does not respond to conventional cardiopulmonary resuscitation (CPR) is poor. We report on survival and early neurological outcomes of children with heart disease supported with rapid-response extracorporeal membrane oxygenation (ECMO) to aid cardiopulmonary resuscitation (ECPR). Methods and Results-Children with heart disease supported with ECPR were identified from our ECMO database.Demographic, CPR, and ECMO details associated with mortali… Show more

“…However, survival statistics for ECPR are more encouraging, with a general rate of success of near 40% to 60%. [2][3][4][5][7][8][9][10][11][12][13][19][20][21][22][23][24][25][26] Multi-institutional data obtained in 2012 from the Extracorporeal Life Support Organization (ELSO), an international registry and database of ECMO treatment, demonstrated that ECPR was successful for 934 out of 2236 neonatal and pediatric patients, with survival to discharge of 39% for neonates and 40% for children. Other retrospective, single-institution studies have shown survival rates as high as 72% to 80%.…”

“…4 Other studies examining ECPR patients have shown favorable neurological outcomes, as measured by the Pediatric Cerebral Performance Category Scale, with a score of 2 or less in most patients. 8,9,19,20,32 Further investigation into long-term neurological sequelae is needed and should be included in future studies.…”

“…Survival for pediatric patients has been reported as anywhere from 23 to 73% based on published, single-center experiences [174][175][176][177][178][179][180][181][182]. Two multicenter registry database analyses have also been performed.…”
