Rapid Intraoperative Localization of Parathyroid Glands Utilizing Methylene Blue Infusion

Abstract: IMBI is a safe, readily available, cost-effective, and underutilized technique that facilitates rapid identification of parathyroid adenomas, helps distinguish normal glands from hyperplastic glands, and helps to locate ectopic glands. An overall reduction in operative time, especially for bilateral neck exploration, can be anticipated.

“…There are no prospective studies published on this topic. Many retrospective studies describe its use [90][91][92][93][94][95][96][97][98][99][100][101][102][103], five of them originating from the same center [104][105][106][107][108]. Two of them showed a small but significant reduction in the duration of surgery [107,109].…”

“…MB aids efficient and rapid identification of abnormal glands in eutopic and, especially, in ectopic locations in the neck. As a result, both the amount of tissue dissection and the operating time have been shown to be significantly reduced with the use of MB, [11][12][13] which must be better and safer for what is a generally older population of patients (as demonstrated in this series, where there was an average age of 60 years with a range of 15 to 89 years).…”

“…To minimize parathyroid injury, many surgical techniques have been developed and a variety of techniques have been used such as intraoperative carbon nanoparticles injections, methylene blue infusion, technetium-99m-sestamibi localization using a hand-held gamma probe, and optical coherence tomography (24)(25)(26)(27).…”
