DiscussionOur results indicate that using the foam results in a good topical application of steroid to the lower rectum but that more proximal spread is limited and infrequent. One criticism that could be put forward is that we made no attempt to ensure that the rectum was clear of any hard faecal matter that might prevent the spread of the foam. All the patients, however, had loose stools, and the foam spread no further in them than in the controls.The aim of the study was simply to assess the extent of spread of the foam after rectal administration and not to attempt to compare the product with any other in terms of extent of spread or efficacy. Certain clinical implications, however, could not be avoided. On the basis of topical application alone the product is a rational treatment for only proctitis and mild distal ulcerative colitis. It would, however, seem illogical to use it for more proximal disease, and our clinical impression has been that under these circumstances it is less effective. If it is of benefit in more proximal disease this can be assumed to be only the result of absorption of the steroid from the foam by the rectal mucosa, any effect being that of systemic steroid. Of the clinical trials quoted by the manufacturer, only that of Scherl and Scherl4 attempted to differentiate between patients with proximal or distal disease. Unquestionably, however, the foam is more comfortable and easier to retain than a retention enema, and since the patient need not be immobilised, the foam obviously has a place in outpatient practice for patients with proctitis and distal ulcerative colitis.We wish to thank Professor B Pullan for his advice, and Stafford Miller Limited for supplying the Colifoam.

Furthermore, the suggested risk factors occurring later in life, such as psychological mechanisms and drug availability and exposition, cannot explain the demonstrated associations between administration of drugs during the perinatal period and subsequent addiction in offspring. Also, genetic and socioeconomic factors were largely controlled for by matching addicts with their own siblings.In conclusion, effective pain relief is sometimes an important factor for a successful outcome of delivery, as well as for the ability of the mother to accept and care for the child in the future. At present, when considering the choice of analgesic method immediate risks and benefits are mainly taken into account. When depressant or sedative drugs are used their possible long term effects due to imprinting also seem to be important. From this point of view, analgesic methods not associated with passage of substantial amounts of drugs across the placenta are preferable. Our Abstract Objective-To see whether a prepregnancy clinic for diabetic women can achieve tight glycaemic control in early pregnancy and so reduce the high incidence of major congenital malformation that occurs in the infants of these women. Design-An analysis of diabetic control in early pregnancy including a record of severe hypoglycaemic episodes in relation to the occurrence of major congenital malformation among the infants.Setting-A diabetic clinic and a combined diabetic and antenatal clinic of a teaching hospital.Patients -143 Insulin dependent women attending a prepregnancy clinic and 96 insulin dependent women managed over the same period who had not received specific prepregnancy care.Main outcome measure-The incidence of major congenital malformation.Results-Compared with the women who were not given specific prepregnancy care the group who attended the prepregnancy clinic had a lower haemoglobin Al concentration in the first trimester (8.4% v 10-5%), a higher incidence of hypoglycaemia in early pregnancy (38/143 women v 8/96), and fewer infants with congenital abnormalities (2/143 v 10/96; relative risk among women not given specific prepregnancy care 7-4 (95% confidence interval 1-7 to 33.2)).Conclusion-Tight control of the maternal blood glucose concentration in the early weeks of pregnancy can be achieved by the prepregnancy clinic approach and is associated with a highly significant reduction in the risk of serious congenital abnormalities in the offspring. Hypoglycaemic episodes do not seem to lead to fetal malformation even when they occur during the period of organogenesis.

Caralluma sarkariae is a succulent, endangered and endemic plant from southern Western Ghats. This species has used by ethanomedicinal practices by traditional healers of Southern Tamilnadu. Traditionally the plant has been used as the treatment of Rheumatism, ulcer, asthma, fever, inflammation, diabetes, stress, cancer and obesity. The methanolic extract of C. sarkariae was investigated for Anti-diabetic activity by STZ induced male wistar albino rats. The result indicated that the methanolic extract at the dose of 150mg/kg b.wt., showed a Significant anti-diabetic activity as compared to the standard drug. The findings of the study suggest that the methanolic extract of C. sarkariae showed significant activities which confirm the traditional usage of this medicinal plant to treat the metabolic disorders.
