“…COVID–19パンデミックに対応すべく各国で仮設病棟が運営されたことが報告されているが,対象患者の重症度は施設ごとに異なる 1) , 2) , 3) 。中国武漢のFangcang shelter hospitalsは,基礎疾患の少ない自立した65歳未満の軽から中等症患者を診療対象として,少ない人的リソースで効率的な診療が提供し得たと報告している 2) 。早期隔離・トリアージ・基本的医療の提供・モニタリングと速やかな後方搬送・生活基盤の提供を5つの主要な機能と位置づけて活動を行い,地域の新規感染者数の低減と既存医療機関の負荷の軽減に寄与した 7) 。一方で,英国ロンドンのNightingale hospitalは人工呼吸器管理を行う重症患者に特化し,重症患者のサージキャパシティの確保に貢献し得たとしている 3) 。神奈川県では,臨時医療施設開設時には無症状および軽症患者の自宅および宿泊施設療養のスキームが既に構築されており,上記2施設の中間にあたる患者層である酸素投与が必要な患者,65歳以上の高齢者および重症化リスクのある基礎疾患を持つ患者,自宅および宿泊施設での療養が困難と判断された患者(精神疾患など)を主たる入院対象とした。機能的にはFangcang shelter hospitalsに近似しているが,より重症化リスクの高い患者層を対象としていた。そのため,臨時医療施設の最優先目標を「重症化抑制」に設定し,①24時間入院応需,②状態悪化の早期察知および早期治療介入,③適時な重症指定施設への転送,の3点を原則とする体制をとった。…”
