Rapid determination of immunosuppressive drug concentrations in whole blood by coated blade spray-tandem mass spectrometry (CBS-MS/MS)

Abstract: Coated Blade Spray (CBS) is a technology that efficiently integrates sample preparation and direct coupling to mass spectrometry (MS) on a single device. In this article, we present CBS-tandem mass spectrometry (CBS-MS/MS) as a novel tool for the rapid and simultaneous determination of four commonly used immunosuppressive drugs (ISDs) in whole blood: tacrolimus (TAC) and cyclosporine-A (CycA), which are calcineurin inhibitors; and sirolimus (SIR) and everolimus (EVR), which are both mTOR (mechanistic target of… Show more

“…31 Analytes absorbed on SPME fibers can be thermally desorbed and then analyzed by GC/MS, eluted in solvent and then analyzed by LC/MS, or directly analyzed by ambient mass spectrometry techniques such as direct electrospray probe, 38 direct analysis in real time (DART), [39][40][41] dielectric barrier discharge ionization (DBDI), 42 desorption electrospray ionization (DESI), 43 wooden tips electrospray ionization (WT-ESI), 44,45 and coated blade spray mass spectrometry. 36,46 Thus, the lengthy pretreatments involved in traditional LC/MS/MS analysis of methylphenidate in blood are no longer required.…”

“…The coating used is not restricted to simple C18 polymer, as demonstrated by the use of hydrophilic lipophilic balance particles and polyacrylonitrile to analyze voriconazole (Tascon et al, ), a mixture of anabolic molecules, β‐2 agonists, diuretics, stimulants, narcotics, and β‐blockers (Tascon et al, ), residual wastewater contaminant pharmaceuticals including paroxetine, fluoxetine, diazepam, and sertraline (Poole et al, ), drugs of abuse such as methamphetamine, buprenorphine, fentanyl, clenbuterol, and oxycodone (Gómez‐Ríos et al, ), and immunosuppressive drugs tacrolimus, cyclosporine‐A, sirolimus, and everolimus (Gómez‐Ríos et al, ).…”

“…The technology for multiplex measurements of drugs and their metabolites are being aided by advancing technologies and are increasingly contributing to clinical care. [ 50–58 ] Mass spectrometry allows for the quantification of therapeutic peptides, [ 59 ] antiepileptics, [ 60–63 ] antidepressants, [ 64 ] antipsychotics, [ 65 ] antituberculosis, [ 66 ] immunosuppressants, [ 50,67,68 ] antimicrobials, [ 69,70 ] cardiovascular drugs, [ 71,72 ] therapeutic kinase inhibitors, [ 73 ] and even monoclonal antibodies. [ 74–76 ] The International Association of Therapeutic Drug Monitoring and Clinical Toxicology has even recognized mass spectrometry assays to be the preferred method of clinical drug quantification.…”
