Rapid cervical phIGFBP-1 test in asymptomatic twin pregnancies: role in mid-pregnancy prediction of spontaneous preterm delivery

Abstract: Objective This study aimed to assess the accuracy of a second-trimester rapid cervical phosphorylated insulin-like growth factor binding protein-1 (phIGFBP-1) test to predict spontaneous preterm delivery in asymptomatic twin pregnancies. Method During the second trimester, a rapid test to detect phIGFBP-1 in cervical secretions was performed on consecutive twin pregnancies between 2009 and 2011, to evaluate its predictive value for spontaneous preterm delivery at <28, <30, <32 and <34 weeks' gestation. Exclude… Show more

“…Of these, 22 were excluded, mainly because they were not a test accuracy study, were duplicate publications, or did not report original data. A total of 43 studies, including 10,203 women, met the inclusion criteria, of which nine provided data on asymptomatic women (n=6131), 36-44 28 on symptomatic women (n=3212), 18,45-71 and six on both asymptomatic (n=452) and symptomatic (n=408) women. 17,72-76 …”

“…44 The remaining study measured the concentration of cervical phIGFBP-1 at 12-13 and 18-20 weeks of gestation. 40 Most studies in symptomatic women assessed the test between 22-24 and 34-36 weeks of gestation.…”

“…To date, there is evidence that both term and preterm labor are linked to an inflammatory process, and over the last few decades several biomarkers, including inflammatory cytokines, have been investigated for the prediction of PTB. Recently, Abbot et al .…”

“…93,94 Overall, the measurement of ph IGFBP-1 had minimal predictive accuracy for preterm birth at <34 weeks of gestation (pooled sensitivity, specificity, and positive and negative likelihood ratios of 16%, 93%, 2.3, and 0.9, respectively). No study has evaluated ph IGFBP-1 in women with twin gestations and threatened preterm labor.…”
