Rapid absorption of sumatriptan powder and effects on glyceryl trinitrate model of headache following intranasal delivery using a novel bi-directional device

Abstract: Administration of sumatriptan intranasally at doses of 10 mg and 20 mg by the breath actuated bi-directional powder delivery device results in rapid absorption. Delivery to target sites beyond the nasal valve induced a similar EEG profile to subcutaneous sumatriptan 6 mg and prevented migraine attacks in patients following GTN challenge. Intranasal administration of sumatriptan powder with the breath actuated bi-directional powder delivery device was well tolerated.

“…(6,7) The delivery was well tolerated and no adverse events were reported. As previously shown for a bidirectional liquid device, (3) this study shows that bidirectional nasal delivery of a powder offers larger and more reproducible initial deposition to the upper and middle target sites with ciliated nasal mucosa found predominantly beyond the nasal valve when compared directly to a traditional hand- …”

“…Upon actuation, the soft palate rises, isolating the nasal cavity from the oropharynx, and a fine powder of sumatriptan is deposited across the entire nasal mucosa on one side and then passes through to the other nostril. Because of the improved deposition of the drug, a phase I study was able to demonstrate that more sumatriptan is absorbed across the nasal cavity than traditional nasal delivery methods [6]. A phase II study was designed to evaluate the efficacy and safety of a single dose of 10 or 20 mg of a powder formulation of sumatriptan delivered with the OptiNose device in comparison with placebo [7].…”

“…In a phase I study, OptiNose proved to have more extensive absorption across the nasal mucosa than the existing sumatriptan liquid nasal spray device on the market [13]. A phase II study was designed to evaluate the efficacy and safety of a single dose of 10 or 20 mg of a powder formulation of sumatriptan delivered with the OptiNose device in comparison with placebo, in 117 adults with a moderate to severe intensity migraine attack [14].…”
