Randomized, Double-Blind Crossover Study to Investigate the Effects of Amlodipine and Isosorbide Mononitrate on the Time Course and Severity of Exercise-Induced Myocardial Stunning

Abstract: Background-Myocardial stunning may cause prolonged left ventricular dysfunction after exercise-induced ischemia that can be attenuated by calcium antagonists in animal models. To assess their effects in humans, we performed a randomized, double-blind crossover study comparing the calcium antagonist amlodipine (10 mg once daily) versus isosorbide mononitrate (ISMN, 50 mg once daily) on postexercise stunning. Methods and Results-Twenty-four men with chronic stable angina and normal left ventricular function unde… Show more

“…The shortening fraction for all 12 segments (SFmean) and in the segments that developed the worst impairment after stress (SFworst) was calculated. Left ventricular volumes at end diastole and end systole were calculated using the biplane disk method, and global ejection fraction was derived as previously described 24. The coefficient of repeatability for this method was 2.6% as assessed by the method of Bland and Altman 25.…”

“…Most patients pretreated with nitrates demonstrated stunning at 24 hours after the angioplasty, whereas only 8% pretreated with nisoldipine showed stunning at 15 minutes after balloon deflation, and all had recovery of left ventricular function by 1 day after reperfusion. Rinaldi et al 15 also studied the effect of calcium channel blockade in patients undergoing exercise stress echocardiograms by comparing the effect of isosorbide mononitrate or amlodipine on postexercise wall motion abnormalities. As expected, shortening fraction was better preserved in patients on amlodipine than in those on nitrates.…”
