Randomized Controlled Trial of Cryoanalgesia (Ice Bag) to Reduce Pain Associated With Arterial Puncture

Abstract: BACKGROUND: Arterial puncture can be a painful procedure for many patients. This study investigates whether precooling of a puncture site with ice can reduce the pain associated with arterial puncture. METHODS: This was a stratified randomized controlled trial of a convenience sample of out-patients with a physician order for an arterial blood gas (ABG) test. The intervention group had a plastic bag of ice applied to their wrists for 3 min before drawing an ABG sample from the radial artery. The control group … Show more

“…However, the difference between two groups of cryotherapy and two control groups was not significant. This study recommended application of cryotherapy as a drug-free, simple, and effective procedure without side effects (Haynes 2015). The reason for this consistency can be related to 20 minutes application of cryotherapy, because for obtaining the desirable physiological effects of cryotherapy, at least 20 minutes of its application is necessary.…”

“…Cryotherapy has been introduced as a noninvasive and nonpharmacological method. It is inexpensive and easily applicable to relieve the pain (Haynes 2015). Various methods of applying cryotherapy include using ice pack, ice massage, cold bath, and cooling gel packs.…”

“…Cooling of the skin is simple and cheap form of anesthesia. The exact mechanism of action of anesthesia by cooling is unknown, but several mechanisms, such as decreased nerve conduction, reduction in muscle spasms, prevention of edema after injury, a decrease in the release of pain-production substances locally, release of endorphins, and pain inhibition through inhibitory interneurons (pain gate), have been suggested [5][6][7][8]. These neural effects increase the patient's pain threshold and can diminish the need for pharmacologic interventions, such as narcotics or local anesthetics.…”
