Randomized Controlled Trial Evaluating Response to Metformin Versus Standard Therapy in the Treatment of Adolescents with Polycystic Ovary Syndrome

Abstract: Adolescents with PCOS treated with metformin or OCP experienced similar beneficial outcomes including reduction in androgen levels, weight loss, and increased insulin sensitivity. The choice of a treatment agent for long-term use will depend on safety profiles, therapeutic goals and patient adherence.

“…81 Metformin is also effective in reducing the incidence of diabetes in those at high risk 80 and has been used with some success in management of symptoms of PCOS in adolescents. [82][83][84] However there is a lack of large, randomized controlled trials to assess efficacy of symptom control in adolescents and no prospective study, even in adults, to examine the long-term effects of metformin in prevention or reduction of metabolic complications of PCOS. Recently, an impressively large and well-conducted trial of metformin as an adjunct to clomiphene treatment for induction of ovulation has shown no benefit of metformin.…”

“…It is not currently approved for use in obesity or insulin resistance in adolescents, and its use for these indications has been 54 which demonstrated a 31% reduction in risk of type 2 diabetes among high risk adults with metformin therapy. Use of metformin to treat obesity-related comorbidities associated with insulin resistance, such as polycystic ovarian syndrome (PCOS) 55 and nonalcoholic steatohepatitis, 56 has also been under investigation. The two small meta-analyses of randomized trials of metformin therapy in pediatrics used overlapping but not equivalent sets of studies and arrived at differing conclusions.…”
