Randomized comparison of cold blood and cold crystalloid renal perfusion for renal protection during thoracoabdominal aortic aneurysm repair

Abstract: Cold renal perfusion during TAAA repair provides effective protection against renal injury. Using cold blood instead of cold crystalloid does not enhance renal protection.

“…228 Despite protective measures such as antegrade mesenteric perfusion, or retrograde perfusion with sequential clamping, mucosal damage can still occur and inflammatory reactions are induced. 229 Risk factors for the development of SCI include prolonged aortic cross clamp time, extensive aneurysmal disease, aortic dissection, emergency surgery, internal iliac artery exclusion, and previous abdominal aortic procedures.…”

“…Renal hypothermia was achieved by selective perfusion of the renal arteries (4 °C lactated Ringer solution with 12.5 g/L of mannitol and 125 mg/L of methylprednisolone). 18 The endograft ( Fig. 2A) was completely removed, and infrarenal reconstruction was performed in end-to-end fashion with use of a rifampin-soaked, silver-coated Dacron bifurcated graft (Fig.…”

“…We transected left renal arteries and infused 20 ml of Ringer's lactate solution with mannitol (12.5 g/L) and methylprednisolone (125 mg/L) cooled to 4°C, which are reported to be protective for renal function. 4) We created two small holes in the aorta with an aortic puncher for coronary artery and anastomosed left renal arteries directly with 7-0 monofilament, nonabsorable continuous suture (Fig. 2).…”
