Randomized Comparison of Artemether-Benflumetol and Artesunate-Mefloquine in Treatment of MultidrugResistant Falciparum Malaria

Abstract: An open, randomized comparison of artemether-benflumetol (CGP 56 697; Novartis) with artesunate-mefloquine was conducted in 617 patients with acute uncomplicated multidrug-resistant falciparum malaria on the western border of Thailand. Both treatments rapidly and reliably cleared fever and parasitemia, and there was no significant difference in the initial therapeutic response parameters. Parasite genotyping was used to distinguish recrudescences from new infections. The 63-day cure rate for artesunate-mefloqu… Show more

“…Unfortunately, halofantrine was found to prolong the QT interval and thus its use is now limited [32]. The most recent amino alcohol, lumefantrine was developed by the Chinese [54] and resembles halofantrine in having a very lipophilic, electron-deficient tricyclic conjugated system. Lumefantrine has solubility-limited oral bioavailability, making the successful combination with artemether a twice daily drug [10].…”

“…Unfortunately, there is a lack of pharmacokinetic information about DBB although it is considered a putative -albeit hitherto elusivemetabolite of lumefantrine. As lumefantrine is tolerated well in therapeutic doses [17][18][19][20], it may be expected that the same will hold for DBB. Recent hERG channel studies with DBB indicate better cardio-tolerability as compared to chloroquine and mefloquine [21].…”

“…The most often used free combinations are artesunate/mefloquine (Artequin) [72,85], artesunate/amodiaquine (Arsucam) [86], artesunate/ sulfadioxine/pyrimethamine (Arsudar) [87], artesunate/ malarone. Efficient fixed combinations are artemether/ lumefantrine (Riamet and Coartem) [88][89][90], dihydroartemisinin-piperaquine (Artekin), artesunate/Lapdap [67].…”
