Although the number of women who survive treatment for colorectal cancer is growing, little is known about the quality of life of long-term survivors. The purpose of analyses presented in this paper is to describe the overall health-related quality of life of female long-term colorectal cancer survivors and the factors that may modify their levels of quality of life. A population-based sample of 726 Wisconsin women diagnosed with colorectal cancer from 1990-1991 was recontacted. Of the 443 women alive in 1999, 307 (69%) completed a follow-up questionnaire including the Medical Outcomes Study Short-Form 36 Health Status Survey, which is comprised of 36 items that generate nine domain scale scores and two summary scores: the Physical Component Summary score and the Mental Component Summary score. The mean follow-up was 9 years (range 7-11), and the mean age at follow-up was 72 years (range 43-85). The mean Physical Component Summary score was lower for participants with greater ages, greater numbers of comorbidities, and greater body masses at the time of follow-up. The mean Mental Component Summary score also was lower for participants with greater numbers of comorbidities. Differences associated with degree of comorbidity were observed for all eight domain scales. Female long-term survivors of colorectal cancer appear to report health-related quality of life comparable with that of similarly aged women in the general population. These data suggest that, over the long term, factors attributable to aging, body weight, and chronic medical conditions play more dominant roles in determining physical and mental health than factors related to the initial colorectal cancer diagnosis. Рак среднеампулярного отдела прямой кишки -какую операцию выбрать? -Низкая передняя резекция прямой кишки КАРАЧУН АЛЕКСЕЙ МИХАЙЛОВИЧ, ПЕТРОВ АЛЕКСЕЙ СЕРГЕЕВИЧРазвитие хирургии последнего столетия позволило перейти от выполнения брюшно-промежностной экстирпации прямой кишки всем больным раком прямой кишки к выполнению большинству пациентов операций, сопрово-ждающихся сохранением сфинктерного аппарата, без ущерба онкологической радикальности. Рассмотрены воз-можности выполнения различных хирургических вмешательств при локализации опухоли в среднеампулярном отделе прямой кишки. RESUMO: O desenvolvimento tecnológico dos últimos anos proporcionou amplas conquistas aplicáveis à Medicina como pouco ocorreu em outras épocas e, no campo da Cirurgia, só teve paralelo na descoberta do controle e domínio da dor e no advento dos antibióticos. Por outro lado, é tão grande o universo do desenvolvimento técnico e tão rápido sua transformação, que nós não temos tempo para avaliar criteriosamente os efeitos de sua transposição para a área médica. A evolução técnica é mais rápida do que sua adequação e uso em benefício da saúde, sobretudo no campo da aplicação das ciências cirúrgicas. As mais otimistas previsões para o próximo futuro deixaram, um pouco de lado, a experiência dos últimos 100 anos em que o extraordinário desenvolvimento tecnológico não proporcionou a contundente revolução que poderia ser esperada. Por exemplo, no campo da Coloproctologia, mormente se nos referirmos ao tratamento e à cura do câncer cólon-retal, o maior benefício foi artesanal, decorrente da destreza cirúrgica na criação da excisão total do mesorreto (ETM), que proporcionou ao paciente menor recidiva local, mais longa expectativa de vida e a preservação da continência anal. Por outro lado, a atual esperança evolucionista para o campo da cirurgia cólon-retal e, provavelmente, para outras especialidades cirúrgicas, denominada de "um progresso sem precedente na história das operações cirúrgicas em que as salas cirúrgicas do futuro não lembrarão, em absolutamente nada, as atuais salas 2 ", desdenha as ocorrências registradas no século passado. Não há dúvidas que esse progresso material há de vir, como ocorreu nos últimos 100 anos, contudo o que nos resta é a dúvida a respeito do custo-benefício e, nesses termos, que valor trará para os nossos pacientes com câncer cólon-retal quando o objetivo estabelecido for a cura da doença maligna? No entanto, não encontramos ao longo desse século, na literatura médica, estudos controlados desenvolvidos por períodos com a finalidade exclusiva de comparar o tempo de sobrevida a partir do tratamento 1 . Dados históricos como esses, cotejados entre si, seriam relevantes porque nos possibilitariam uma visão mais ampla dos resultados e nos permitiriam a construção de um perfil mais fiel do que tem, objetivamente, sido feito com a aplicação dos conhecimentos teóricos e das técnicas modernas disponíveis para o tratamento do câncer cólon-retal. Abstract: Patients undergoing low anterior rectal resection and coloanal J pouch reconstruction may expect not only better functional results but also an improved quality of life in the early months after surgery compared with patients who receive a straight coloanal anastomosis.
