Background and ObjectivePulsed dye laser (PDL) has been reported to be safe and effective in the management of superficial hemangiomas of infancy. We report 12 patients with hemangiomas with complications following PDL.Study Design/Materials and MethodsRecords of patients with hemangiomas and a known adverse outcome following PDL were reviewed.ResultsAll were treated early (age range: 5 days to 4 months), and all hemangiomas were facial with a superficial component. Eleven were treated with a 585 nm wavelength, fluence range of 4.7–7 J/cm2, without dynamic cooling. One patient received 7–12 J/cm2 utilizing a 595 nm wavelength with dynamic cooling. In eight cases, treatment led to severe ulceration with subsequent pain, scarring, and in one instance, life‐threatening hemorrhage. In four, permanent atrophic scarring was noted without ulceration.ConclusionsPDL treatment of superficial hemangiomas may rarely lead to significant complications including atrophic scarring and severe ulceration. Özet Sum maryLaser treatment of cutaneous vascular lesions has progressed significantly over the past decades. Lasers and intense pulse light sources (IPL) are based on the principle of selective photothermolysis and can be used for the treatment of many vascular skin lesions. Light devices for vascular lesions include the 532 nm potassium titanyl phosphate laser, 577 to 595 nm pulsed dye laser, intense pulsed light devices, and 800 to 940 nm diode, 755 nm alexandrite and 1,064 nm Nd:YAG lasers. The targeted chromophore for vascular lesions is intravascular oxyhemoglobin. By properly selecting the wavelength which is maximally absorbed by the target and a corresponding pulse duration the target can be preferentially injured without transferring significant amounts of energy to surrounding tissues. Various congenital and acquired lesions can be treated such as hemangiomas, port wine stains, facial telangiectasia and leg veins. Cooling will prevent adverse effects such as pigment alteration and scar formation during therapy. A different number of repeated treatments should be done to achieve complete results of different vascular conditions. This review will discuss the different laser and light-based devices, and provide treatment approach for the management of vascular lesions. Gi riflVasküler lezyonların lazer ile tedavisi dermatoloji pratiğinde en sık uygulanan yöntemlerden birisidir. Son yıllarda lazer teknolojisindeki gelişmelere paralel olarak lazerle tedavi edilebilen vasküler lezyonların spektrumu da genişlemiş ve birçok konjenital ve akkiz vasküler lezyon farklı lazer sistemleri ile tedavi edilebilir hale gelmiştir. Selektif fototermoliz prensibi ile çalışan bu cihazlarda, hedef kromofor tarafından absorbe edilen ışınlar dokularda istenilen biyolojik etkileri ortaya çıkarırken çevre dokuların korunmasına olanak tanımakta, krutlanma, kanama veya skar oluşumu gibi istenmeyen yan etkiler daha az oluşmaktadır. İntravasküler oksihemoglobin (kırmızı lezyonlar), deoksijene hemoglobin (mavi lezyonlar) ve methemoglobin vasküler lezyonların tedavisinde hedeflenen kromoforlardır.
