Summary Eighteen patients with advanced cancer have been treated intravenously with human recombinant tumour necrosis factor (rhTNF). The drug produced febrile reactions at all doses although these were preventable by steroids and indomethacin. Doses at or above 9 x 105 units (400 pg)m-2 were associated with hypotension, abnormal liver enzymes, leucopenia and mild renal impairment in a substantial proportion of patients. RhTNF was cleared from plasma with a half life of -20 minutes but non-linear pharmacokinetics were seen with decreased clearance at higher doses. In 3 patients, all with lymphoma, improvements in their tumours were recorded.RhTNF was noted to produce rapid increases in serum C-reactive protein concentrations. Endogenous TNF levels were not found to be elevated in 72 cancer patients.TNF deserves further therapeutic evaluation and these observations support its biological importance as an endogenous pyrogen, mediator of acute phase protein responses, and a mediator of endotoxic shock.

Flavonoids are potent antioxidants and have been associated with lowering the risk of cardiovascular diseases. In this study, the effect of flavonoids (monomers, dimers and a trimer) as well as French maritime pine bark extract, Pycnogenol, on NO production, tumor necrosis factor-K K (TNF-K K) secretion and nuclear factor (NF)-U UB activity was compared. Monomers and dimers repressed NO production, TNF-K K secretion and NF-U UB-dependent gene expression induced by interferon Q Q, whereas the trimeric procyanidin C2 and Pycnogenol enhanced these parameters. In addition, in unstimulated RAW 264.7 macrophages, both procyanidin C2 and Pycnogenol increased TNF-K K secretion in a concentration-and timedependent manner. These results demonstrate that procyanidins act as modulators of the immune response in macrophages.z 2000 Federation of European Biochemical Societies.

Changes in lipid plasma levels during malaria attacks have been proposed for use in diagnosis or to assess the severity of the disease. In order to analyse the plasma levels of cholesterol, triglycerides, HDL-C et LDL-C, we compared, in an endemic area (Sao Tome island), two groups of patients children infected with Plasmodium falciparum (simple malaria attack and cerebral malaria) with a control group of asymptomatic children. No correlation between lipid plasma levels and disease severity was found. Cortelations between lipids and parasitemia or anemia were analysed.
