Radiotherapy of regional epidermoid carcinoma of the lung: A study in fractionation

Abstract: This paper reports a prospective randomized multicenter trial of two fractionation schemes in the supervoltage radiotherapy of locally inoperable epidermoid lung cancer. This study was stimulated by the need to provide improved and more acceptable treatment methods for patients with this condition, the most common cell type of lung cancer. The majority of such patients are not suitable for resection because of metastasis or because of extensive local disease, even though exploratory thoracotomy may be performe… Show more

“…More than one‐third of NSCLC patients presented with locally advanced tumors, without distant metastatic disease, which could not be primarily resected . Radiotherapy is usually used as the primary local treatment but has associated unsatisfying survival rates . Locally recurrent tumors and their complications account for about 75% of the deaths in patients with epidermoid lung cancer, and approximately 40% of the deaths in patients with other histologic subtypes of NSCLC .…”

“…16 Radiotherapy is usually used as the primary local treatment but has associated unsatisfying survival rates. [17][18][19][20] Locally recurrent tumors and their complications account for about 75% of the deaths in patients with epidermoid lung cancer, and approximately 40% of the deaths in patients with other histologic subtypes of NSCLC. 21,22 To have an impact on the overall outcome of the management of patients with localized NSCLC, one should make an attempt to decrease the probability of local failure as well as distant metastasis.…”

“…SQCLC indicates expanded expression of ligands for PD-1 known as PD-L1. Blocking either the receptor or ligand could defeat invulnerable resistance prompting tumor relapse [59][60][61][62][63]. Nivolumab is a human IgG4 PD-1 antibody agent which was affirmed for second line treatment of metastatic SQCLC in April 2015.…”

“…The mortality rate of lung cancer is very high and almost 1 in 5 cancer patients [1,2]. There are 2,24,210 new cases and 1, 59,260 deaths are reported in the year 2014 [3]. Basically there are two major types of lung cancer viz.…”

“…Thus, it was the current treatment of potentially curable lung cancer at the Mayo Clinic (Lee, 1983). The side effects of this short course radiation regimen were compared to those of a less intensive schedule, delivering 50Gy in 15 fractions in 3 weeks plus 10 fractions in 2 weeks, with 2 weeks rest (Sealy et al, 1982). The toxicity was acceptable in both treatment groups and no significant differences were observed.…”
