Radiomics analysis of multiparametric MRI for prediction of pathological complete response to neoadjuvant chemoradiotherapy in locally advanced rectal cancer

Abstract: • Radiomics analysis of pre-CRT multiparameter MR images could predict pCR in patients with LARC. • Proposed radiomics signature from joint T2-w, ADC and cT1-w images showed better predictive performance than individual signatures. • Most of the clinical characteristics were unable to predict pCR.

“…Our study was in general consistent with prior researches. Nie and Cui reported a relatively satisfactory result by using a radiomics method, with AUCs of 0.84 and 0.94 for pCR prediction, respectively. However, there was an obvious predominance in our study compared to their studies.…”

“…After CRT, rectal cancers demonstrate variable degrees of tumor response, including pathologic complete response (pCR) in 15% to 27% of patients. [ 1 ] Patients with complete response (CR) or tumor downstaging after neoadjuvant CRT can achieve excellent local tumor control and a better quality of life, and organ-preserving treatments, such as watch-and-wait approach or local excision can be performed instead of TME. [ 2 , 3 ]…”

“…Approximately 20 percent of the rectal cancer patients have a good response to neoadjuvant chemoradiotherapy and the tumor disappear completely. This phenomenon is called clinical complete response(cCR) [5]. The stringent definition of cCR was proposed by Mass in 2012, and it contained five diagnostic criteria.…”
