Radiology of Severe Acute Respiratory Syndrome (SARS)

Abstract: Severe acute respiratory distress syndrome (SARS) caused by SARS-associated coronavirus (SARS-CoV) is a systemic infection that clinically manifests as progressive pneumonia. During the initial phases of infection the virus causes pauci-inflammatory alveolar and interstitial edema that result in imaging abnormalities dominated by ground glass opacities (GGO). Severe SARS cases can develop radiologic and pathologic findings of diffuse alveolar damage. Although radiologic evidence of acute bronchiolitis is absen… Show more

“…The main lung pathology in SARS and MERS is DAD in various stages, which is basically the histological prototype of acute lung injury [43,44]. According to the previous pathological mechanism of SARS, the emergence of GGO suggests that SARS-CoV-2 causes airspace exudates, and pauci-inflammatory alveolar and interstitial oedema [45]. Crazy paving appearance consists of GGO with superimposed interlobular and intralobular septal thickening, reflecting interstitial lesions [46].…”

“…In view of the apparent lack of success thus far with antiviral therapies and the time needed to develop vaccines, we propose strategies that focus on blunting the pathobiology that we hypothesize results from ACE1 (angiotensin-converting enzyme 1)/ACE2 imbalance and enhanced Ang II signalling that occurs with SARS-CoV-2 infection. Figure 2 shows a schema of the RAS pathway and is based on data for angiotensin signalling from a range of sources (Forrester et al, 2018;Karnik, Singh, Tirupula, & Unal, 2017;Santos et al, 2019;Tikellis & Thomas, 2012), including the Guide to Pharmacology database (GtoPdb; https://www.guidetopharmacology.org/) (Armstrong et al, 2020). In the 'traditional'/canonical RAS pathway, renin is secreted from the kidney, and angiotensinogen (AGT) is produced and secreted by the liver.…”

“…ACE2, a carboxypeptidase (zinc metalloprotease), is the primary enzyme responsible for Ang II degradation, thus regulating signal transduction by Ang II. The conversion by ACE2 of Ang II to Ang (1-7) (and its signalling via MAS1) produces effects that oppose those of Ang II (Karnik et al, 2017;Santos et al, 2018;Santos et al, 2019).…”

“…During the next phase of infection (weeks 2e5), the lungs display signs of fibrosis, in which epithelial cells and alveolar spaces show fibrin deposition and infiltration of inflammatory cells and fibroblasts. During the final stage (weeks 6e8), pulmonary tissue becomes fibrotic with collagen deposits, and cellular proliferation is seen in alveoli and interstitial spaces (Cheung et al, 2004;Gu and Korteweg, 2007;Ketai et al, 2006). Radiographic features of patient's lungs varied greatly by individual however characteristic features were present in most patients including progression from unilateral focal air-space opacity to multifocal or bilateral consolidation in the later phases of disease.…”
