A 33-year-old nulligravid Filipino woman was admitted to the gynaecology ward of Princess Margaret Hospital in April 2014, with lower abdominal pain and recurrent retention of urine. Her pregnancy test was positive. Per-vaginal examination revealed a 4-cm firm, fixed, and wide-based vaginal mass with smooth wall over the upper anterior vagina. Bedside transabdominal ultrasonography confirmed a 4.5-cm solid mass in the anterior vagina and a 3. <5.0 IU/L for non-pregnant women), and repeated checking 3 days later was 253 957 IU/L. Alphafetoprotein, carcinoembryonic antigen, and Ca-125 were negative. A urologist was consulted and the patient underwent flexible cystoscopy that revealed indentation of the urinary bladder by an external mass. Mild erythematous changes were visualised at the right urinary bladder base. Biopsy revealed inflammation but no malignant cells.Magnetic resonance imaging (MRI) of the pelvis revealed a 4.8 cm x 5.2 cm x 4 cm (craniocaudal x anteroposterior x transverse) mass located at the anterior aspect of the vagina. Background: Choriocarcinoma is a rare, aggressive subtype of gestational trophoblastic neoplasia. The diagnosis of metastatic choriocarcinoma in the setting of a viable intrauterine pregnancy is exceedingly rare and often associated with feto‐maternal hemorrhage. Case: An otherwise healthy Gravida 1 Para 0 at 34 weeks gestational age presented with metastatic choriocarcinoma and a viable fetus. Measured Doppler peak systolic velocity of the middle cerebral artery was used to detect fetal anemia, thus optimising the timing of delivery. Conclusion: This is the first case report to our knowledge using Doppler ultrasonography to detect fetal anemia in an effort to guide delivery in a case of choriocarcinoma diagnosed during pregnancy. If choriocarcinoma is diagnosed during pregnancy, middle cerebral artery Doppler ultrasonography may serve as a critical tool to help detect anemia, allowing pregnancy prolongation to promote fetal maturity while screening for the development of feto‐maternal hemorrhage.

Gestational trophoblastic neoplasms (GTN) originate from placental tissue and usually metastasizes because of its propensity for early vascular invasion. Intracranial metastases are relatively uncommon. There are limited data on the efficacy of 18-fluorodeoxyglucose positron emission tomography (FDG-PET) in the evaluation of metastatic GTN. We present a case of GTN with simultaneous pulmonary and cranial metastases in which the cranial metasteses were undetectable with positron emission tomography (PET) scan.Key Words: Gestational Trophoblastic Neoplasms, Cranial Metastasis, Positron Emission Tomography Gestasyonel Trofoblastik Neoplaziler (GTN) plasenta dokusundan köken alırlar ve erken vasküler invazyon yaptıklarından sıklıkla metastaz yaparlar. Beyin metastazı nadir görülür. Metastatik GTN'nin taranmasında 18-florodeoksiglukoz pozitron emisyon tomografisinin (FDG-PET) etkinliği konusunda veriler yetersizdir. Bu olgu sunumunda, eş zamanlı akciğer ve beyin metastazı olan ve beyin metastazının FDG-PET ile saptanamadığı bir GTN olgusunu sunuyoruz.
