Radiologische Frühdiagnostik der skapholunären Dissoziation (SLD)

Abstract: The partial tear of the scapholunate ligament (pre-dynamic stage of SLD) as well as the complete tear (dynamic stage) does not lead to carpal malalignment. However, if the completely ruptured ligament is accompanied by lesions of the extrinsic ligaments, both the scaphoid and the lunate are malaligned already at rest (static stage of SLD). Later, osteoarthritis will develop, beginning in the radioscaphoid compartment, progressing to the midcarpal joint, and ending in a carpal collapse (osteoarthrotic stage of … Show more

“…The scaphoid shift test is a commonly used clinical test for the diagnosis of scapholunate instability [ 3 , 14 , 16 ]. The aim of the present study was to evaluate the diagnostic significance of this test in a large cohort of patients in a clinical setting.…”

“…The scaphoid shift test or “Watson test” is a widely used clinical test that can be used for the clinical diagnosis of SLL instability since the manoeuvre provokes a subluxation of the scaphoid over the dorsal rim of the radius [ 12 , 14 , 15 , 16 ].…”

“…The best known indices are a widened SL joint space and abnormalities of the carpal angles [3][4][5]. Other radiologic features like disruption of carpal arc alignment, a foreshortened scaphoid, loss of SL articular parallelism or asymmetric widening and a dorsally or subdisplaced scaphoid onto the dorsal rim of the radius in lateral projection have also been associated with this condition and may indicate ligament disruption [6][7][8][9]. Contralateral radiographs are suggested as a simple reference method for intraindividual comparison of common indices of wrist physiology and pathology [10][11][12].…”
