Radiological improvement by tocilizumab in polyarticular juvenile idiopathic arthritis

Abstract: Recent advances in biologic therapy have enabled reduction of the progression of destructive arthritis in rheumatoid arthritis. Once destroyed, however, the affected bones and cartilage are not fully repaired. We describe the case of an 8-year-old girl with anti-citrullinated peptide antibody (ACPA)-positive polyarticular juvenile idiopathic arthritis (p-JIA). Destructive arthritis progressed during combination therapy with infliximab, methotrexate, mizoribine and prednisolone. Clinical remission was achieved,… Show more

“…A retrospective observation study found tocilizumab had a risk of progressive joint destruction in patients who retained abnormal MMP-3 values even after achieving clinical remission and in cases with an older age of onset [66]. Meanwhile, one radiological repaired case following tocilizumab treatment was reported in poly-JIA [67].…”

“…Erosion repair is mainly shown on HR-CT [67,68] or MRI scans [63] in research setting, see Table 1. Only a few case reports of mainly systemic juvenile idiopathic arthritis patients treated with tocilizumab demonstrate erosion repair on conventional radiographs [81][82][83][84] however no large-scale research trial has demonstrated significant erosion repair with IL-6 inhibition on X-rays. Although the clinical significance of erosion repair seen on HR-pQCT is unclear, these findings may help to stratify treatment in patients at high risk of erosive progression.…”

“…Eine sichere Unterscheidung zwischen gesund und krank ist im Einzelfall erschwert. Die dynamische Kontrast-verstärkte Darstellung eignet sich sehr gut zur vergleichenden Darstellung und somit auch zur Dokumentation eines Therapieerfolges (7,12,22). Neben dem Achsenskelett erfolgt bei der JIA die Darstellung der Kiefergelenke sowie von Handund Fußwurzel häufig durch eine MRT.…”
