Juvenile idiopathic arthritis (JIA) is a chronic childhood arthritis. Its pathogenesis is very complicated, with the involvement of not only immune cells but various types of parenchymal cells, and is affected by both genetic and environmental predispositions. The clinical spectrum from inflammation to related conditions is largely mediated by cytokines including interleukin (IL)-6. Fluctuations in IL-6 and its related molecules can modulate the pathogenesis and the clinical presentation positively or negatively. The recent clinical impact of IL-6 blockade on JIA has begun a therapeutic paradigm shift. This review describes the characteristics of JIA, mainly focused on IL-6 with the current therapeutic perspective.

The therapeutic armamentarium for rheumatoid arthritis has increased substantially over the last 20 years. Historically antirheumatic treatment was started late in the disease course and frequently included prolonged high-dose glucocorticoid treatment which was associated with accelerated generalised bone loss and increased vertebral and non-vertebral fracture risk. Newer biologic and targeted synthetic treatments and a combination of conventional synthetic DMARDs prevent accelerated systemic bone loss and may even allow repair of cortical bone erosions. Emerging data also gives new insight on the impact of long-term conventional synthetic DMARDs on bone health and fracture risk and highlights the need for ongoing studies for better understanding of “established therapeutics”. An interesting new antirheumatic treatment effect is the potential of erosion repair with the use of biologic DMARDs and janus kinase inhibitors. Although several newer anti-rheumatic drugs seem to have favorable effects on bone mineral density in RA patients, these effects are modest and do not seem to influence the fracture risk thus far. We summarize recent developments and findings of the impact of anti-rheumatic treatments on localized and systemic bone integrity and health.

ZusammenfassungKennzeichen der juvenilen idiopathischen Arthritis sind neben der synovialen Entzündung knorpelige und knöcherne Veränderungen. Während der vergangenen Dekade ist es, insbesondere durch den zunehmenden Einsatz von Biologika, zu einer deutlichen Verbesse-rung der Therapieeffektivität gekommen. Als Folge dieser Entwicklung wandelten sich die Anforderungen an die bildgebenden Verfahren von der reinen Dokumentation von Schäden, hin zur Entdeckung früher Krankheitszeichen. Da diese der konventionellen Röntgendiagnostik verborgen bleiben, spielen empfindlichere Methoden wie die Kontrastverstärkte Kernspintomografie oder der Dopplerultraschall eine zunehmende Rolle bei der Diagnose, Risikostratifizierung und Verlaufskontrolle. Allerdings haben Röntgendiagnostik und die Computertomografie weiterhin eine Bedeutung zum Ausschluss von Frakturen, Tumoren, in der Dokumentation knöcherner Veränderungen sowie in der sicheren intraartikulären Steroidinjektion in schwer zugängliche Gelenke.
