Radiological features of childhood giant cavernous malformations

Abstract: In children, a GCM should be considered in case of very large hemorrhagic intra-axial mass with "bubbles of blood" multicystic appearance, surrounding hemosiderin ring, fluid-fluid levels, and accompanying edema-mass effect, especially in the periatrial location.

“…The reported MRI features of GCMs include non-enhancing multi-cystic lesions with mixed intensity and a surrounding hemosiderin ring, large masses with a "salt and pepper" appearance, and spherical lesions consisting of acute and subacute hematomas. 24 The presentation after contrast injection is highly inconsistent, ranging from nonexistent to intense enhancement. 9,14 GCMs can also appear as complete solid masses and be misdiagnosed as neoplasms.…”

“…Their clinical presentation depends on size and location, and is affected by recent episodes of bleeding. GCM occur more often in children and adolescents (12). In children they are frequently associated with intractable epilepsy (13,14).…”

“…Histologically, large CM do not differ from the smaller ones, and only the diameter and radiological characteristics differentiate them 2,3,11 . Although they are much more common in the supratentorial portion, they can occur in any region of the neuroaxis.…”

“…Radiological findings that can raise the diagnosis of a large cavernoma include the presence of a large intra-axial hemorrhagic mass, with multicystic appearance, surrounding hemosiderin ring, fluid-fluid levels with edema and mass effect 3 . These characteristics are quite variable, making it initially misdiagnosed as brain tumors or other vascular malformations.…”
