Radiological evaluation of covered self-expandable metallic stents used for palliation in patients with malignant esophageal strictures

Abstract: Fluoroscopic placement of self-expandable metallic stents is a safe and effective method of palliating severe dysphagia and fistulas in patients with inoperable esophageal carcinoma. However, complications such as tumor overgrowth and food impaction may require reintervention after stent placement.

“…SEMSs have been successfully used since the early 1990s for palliation of primary esophageal cancer. 13,[19][20][21][22][23] However, the efficacy of SEMS therapy in patients with recurrent cancer after esophagectomy is not well established. The current cohort study, which is the largest published to date, shows a clear benefit of SEMS therapy in patients with recurrent tumor growth after esophagectomy.…”

“…In the present study, a high success rate of stent placement was evident, with adequate relief of dysphagia in most patients. The technical success and symptom improvement rates were comparable to patients without previous surgery 10,11 . Food bolus obstruction after stenting accounted for most of the re‐intervention episodes.…”

“…However, migration rate is higher in coated stents. In light of these information, semi-coated metallic stents were preferred for malignant upper gastrointestinal tract stenoses in this study (5)(6)(7).…”

“…In upper gastrointestinal tract stenoses, stent procedures are minimally invasive procedures that are easy to perform and lead to rapid improvement in dysphagia symptoms. However, they can lead to complications with high morbidity and mortality rates such as bleeding, perforation, retrosternal pain, aspiration pneumonitis, arrhythmia, stent migration, stent occlusion, and tracheoesophageal fistula (6). Complication rates in stent placement without fluoroscopy are around 5% (4).…”
